AFFIDAVIT OF NON-FILER
I, JANET B. THOMAS, of legal age, widow, Filipino Citizen and resident
of Sitio Baguio Gold, Tuding, Itogon, Benguet, Philippines, after having been
duly sworn to in accordance with law hereby depose and state that:

1. That I am the biological mother of CHINY BUGNAY THOMAS and

applied for her Scholarship Grants;

2. That I am the only one raising her and taking good of her due to the
fact that her father died sometime in September, 2016;

3. That among the requirements needed for the said scholarship is a

TAX EXEMPTION CERTIFICATE from the Bureau of Internal
Revenue (BIR);

4. That I am a marginal farmer and occasionally seasonal farm laborer

earning an annual income which does not exceed the annual poverty
threshold level as determined and provided by the National Economic
Development Authority (NEDA). Considering that my joint income is
lower than the annual poverty threshold I do not file Income Tax
Return (ITR);

5. That I am executing this Affidavit to attest to the truth of the foregoing

facts and to support my application for issuance of TAX EXEMPTION
CERTIFICATE and for all legal intents and purposes it may serve.

IN WITNESS HEREOF, I have hereunto set my hand this 4th day of
