AFFIDAVIT

I KAREN DUTOSME DUAY of legal age, Filipino married to Albertini P. Duay

and residing in Poblacion, Sulop, Davao del Sur in accordance with law hereby depose
and say;
That I am the same person who caused for the execution of this instrument.
That I am a buyer of a parcel of land known as lot no. 228, PLS-632 containing an
area of sixty one thousand five hundred eighty one (61,581) square meters situated in the
Barrio of Dalay (now Lapla) Municipality of Sulop, (now part of Municipality of
Malalag) Davao del Sur covered by Original Certificate of Title no. 19119)P-3171 in the
name of AMBIT TAMBIS married to Magdalena Marcelo.

That my ownership and possession of the above-described parcel of land is

evidenced by a Deed of Absolute Sale dated May 12, 2022 executed by IRENIO
CAMBARIJAN and entered in the Notarial Registry of Atty. Victoria Dalisan Ababa
with Doc. No. 463, Page no. 73, Book no.38 Series of 2022.

That IRENIO R. CAMBARIJAN’s previous ownership of the above-mentioned

parcel of land is evidenced by a Deed of Absolute Sale executed by MANUEL
CAMPANER dated December 11, 2017 and entered in the Notarial Registry of Atty.
Victoria D. Ababa with Doc. no. 696, page no; 124, Book no.25 series of 2017.

That MANUEL CAMPANER’s previous ownership on the above-described

parcel of land is evidenced by a Deed of Absolute sale dated 27 th day of December, 1972
executed by AMBIT TAMBIS and Magdalena Marcelo and entered in the Notarial
Registry of Notary Public Francisco Borja with Doc. No.222, Page no.;4 ;Book no. v
series of 1972.

That in the Book of Assessment in the office of the Municipal Assessor of

Malalag, the existing record of Tax Declaration is in the name of MANUEL
CAMPANER with entries the same with the entries such as lot number survey number
and boundaries reflected in OCT no. 19119) P-3171 of AMBIT TAMBIS.
 That I executed this Affidavit for the purpose of securing a tax declaration in the
name of AMBIT TAMBIS and cancel tax declaration of Manuel Campaner for the
Subject parcel of land to comply with the requirements of the Bureau of Internal

Revenue, Department of Agrarian Reform and Register of Deeds that entries and data in
the title and tax declaration such as tittle’s registered name and declarants name and other
entries should be the same, in order for the transfer of Title to my name be given due
course by said government offices.
That further I executed this Affidavit to Apprise the Municipal Assessor of
Malalag and The Provincial Assessor of Davao del Sur and other concerned authorities as
to the veracity and truthfulness of the foregoing facts.
