AFFIDAVIT OF LOSS

I, PEDRO C. ROA JR. of legal age, Filipino, married and residing in Ruparan,
Digos City in accordance with law hereby depose and say;

That I am the same person who caused for the execution of this instrument.

That during the time my mother Rosita Castel Roa was already in her deathbed
sometime in 2018 I was told by her that all the documents related to the properties left by
our deceased father including the Transfer Certificate of Title no. T-17329 for the
property in Ruparan, Digos City were placed in her old wooden closet.

That after the burial of my mother I scanned the documents inside the old wooden
closet left by her and I was able to find the Tax Declarations real property receipts of tax
payment, survey plans and other documents related to the unregistered lands left by my
parents.

That I also tried to find the title for the property in Ruparan, Digos City but
despite diligent efforts to locate it even in other shelves inside the closet of my mother,
the same could not be found.

That I tried and contacted my brother and sisters if the Transfer Certificate of
Title no.T-17329 for the property in Ruparan, Digos City, was in their position but their
reply is negative.

That I executed this Affidavit to appraise concerned authorities as to the veracity

and truthfullness of the foregoing facts.
