JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, EMILIO A. ACOSTA, of legal age, Filipino, a resident of #G. Reyes City, Pasay City and
RYAN B. MIRANDA, of legal age, Filipino, a resident of Unit 14D Blk Lot 4 Southern Yemen St.,
Betterliving Subd, Parañaque City, after having been duly sworn to in accordance with law,
hereby depose and state:
1. That we personally know RACHEL JOSEFA C. GARCIA, being a neighbor at Chateau
Elysee Condominium, Dona Soledad Extension, Betterliving Subd, Parañaque City;
2. That we have a personal knowledge of the above-facts because the above-named
which had been our long time friends;
3. That we learned that RACHEL JOSEFA C. GARCIA was previously registered in her Birth
Certificate in the PSA as RACHEL JOSEFA C. GARCIA, as Annex “A” and that in her school
records show as RACHEL C. GARCIA, such as her school diploma as Annex “B”;

4. That the names RACHEL JOSEFA C. GARCIA, and RACHEL C. GARCIA refer to one and
the same person;

5. That we are not related by blood or by affinity with the above- named family and that
we are not whatsoever interested in the above-stated facts;

6. That we are executing this Affidavit to attest to the truthfulness of the foregoing and
for all legal intents and purposes.
