ereby solemnly affirm and state on oath as follows:-

1) That I have applied for Site constructed by the Karnataka Housing Board under
category at Mysuru Dist., Mysuru Taluk, Rammanahalli Village, demand survey date
12 -2017 of KHB project at Mysuru District . I have accepted your terms and condit
contained in the Notification.

2) I affirm that I nor my wife/Husband or my minor children own or process site, house o
anywhere in urban area/ Municipality of Karnataka State.

3) I further reaffirm that I have made my application for allotment of Site under MIG Catego

4) I further reaffirm that no house or Site has been allotted to me or to my wife /husban
Karnataka Housing Board or any authority in the state of Karnataka.

5) I further reaffirm that I am staying in Karnataka since by birth.

6) If any of the statements that I have made in my application and in this affidavit is found
incorrect , the Karnataka Housing Board shall be at full liberty to reject the applicatio
cancel the allotment and resume the property.
