I am writing to you on behalf of the Plaridel Community Hospital regarding an urgent matter
concerning the Typhoid test kits that were supplied to us by your company.

We deeply appreciate your commitment to providing us with essential medical supplies,

including the Typhoid test kits that have been instrumental in ensuring the health and well-
being of our patients. However, I regret to inform you that we have recently discovered that
three boxes and additional 27 test kits are about to expire on September 18, 2023.

In light of this, we kindly request your assistance in replacing the aforementioned 3 boxes
plus 27 test kits of Typhoid test with new supplies that have a longer shelf life. This will not
only ensure that we can continue to provide quality healthcare services to our patients but
also maintain the hospital's reputation for excellence.

We trust that you will take our request into consideration and endeavor to resolve this issue
as soon as possible. Your prompt action in this matter will be highly appreciated and will
