1. To expedite the processes of addressing LIS and BEIS issues and concerns
encountered ty the School Heads, LIS and BEIS School Coordinators and
School ftystem Administrators, this Office, through the School Governance
and Operations Division, establishes the LIS and BEIS Helpdesk of SD0
Tuguegarao City. This helpdesk is an online form and can be accessed via
the link below:

2. The SDO Tuguegarao City LIS/BEIS Helpdesk shall be the official platform
used to submit all SERVICE REQUESTS, INCIDENT REPORTS, and
INQUIRIES that need to be forwarded for appropriate actions in the LIS
Helpdesk Ticketing managed by the Information and Communications
Technology Service (ICTS) through the User Support Division (USD) of the
DepEd Central Office.

3. The different 8BHVICB REQt]ESTS shall be limited to the following:


• JanJ LRJVJrcrgzpg -A process of combining two or more leamers'
data with conflicting information pertalhing to a specific leamer
through his/ her official documents subniitted to the school.
• ROB IRJ\r Reactztiatton - A process of re-activating LRN that was
previously disapproved by the Division Planning Officer.

functionality of the system.

5. INQUIRES cover the processes of asking a question.

6. The following requests/data coITections LISTED BELOw are HOT


COVERED by this helpdesk for these require ONLY the approval of the
Division Planring Officer (DPO). These requests/data corrections are
directed to the DPO's LIS account for appropriate action/s once
created/requested through the IJS account of the School Head/LIS System
Administrator/ Class Adviser:
• LEN Approval
• Correction ofl.earner profile (including ALS)
• Correction of Grade lfvel
• Enrolment of lneligible
• Enrolmentwith Gap
• Pending Transferred-In and Transferred-Out
• Temporary Enrolment -refer to DepEd Order No. 3 s. 2018 for the
required affidavit of undertaking
7. To properly address these requests/data conections not covered by the
Helpdesk, you are required to submit evidence/s (i.e., birth certificate,
assessment result, report card, ctc.) to support your requests. Submission
of evidence/s shall be done online through uploading scarmed copies to
your designated folder in the google drives below:

8. The access to this online form will be shared to the School Heads, LIS and
BEIS School Coordinators and School System Administrators OHILY.

9. Following the escalation procedure of IcTS-USD, all issues, concerns, and


requests received from schools through the SDO Tuguegarao City
LIS/BEIS Helpdesk shall be reviewed and forwarded to the LIS Helpdesk
Ticketing managed by the ICTS-USD of the DepEd Central Office through
the Planning and Research Section, this Division, for appropriate action/s.
