Requirements: Domestic Adoption

1. Qualifications of adopter:

a. Filipino citizen;
b. of legal age;
c. in possession of full civil capacity and legal rights;
d. of good moral character;
e. has not been convicted of any crime involving moral turpitude;
f. is emotionally and psychologically capable of caring for children;
g. at least sixteen (16) years older than the adoptee;
h. is in a position to support and care for his children in keeping with the means of
the family; and
i. has undergone pre-adoption services as required by Section 4 of Republic Act
No. 8552;

Proof:

i. Birth certificates of adopters (PSA/NSO);


ii. Marriage Certificate of adopters (PSA/NSO);
iii. Clearances:

1. NBI;
2. Barangay Clearance;
3. Police Clearance;
4. Provincial Prosecutor’s Office;
5. City Prosecutor’s Office;
6. MCTC – Dauis-Panglao;
7. RTC – Bohol (multiple sala);

iv. Certification from DSWD as to attendance to Pre-adoption Seminar;


v. Medical Certificates of adopters;
vi. ITR/Proof of Income/Bank Accounts;

2. Qualifications of adoptee:

a. A person of legal age regardless of civil status, if, prior to the adoption, said
person has been consistently considered and treated by the adopters as their
own child since minority;

Proof:

i. Birth certificate/s of adoptee;

1. Ayeng; and
2. Dumangcas;

ii. Baptismal certificate;


iii. School Records/Report Cards (pre-school, elementary, high school);
iv. Diploma/TOR;
v. IDs/Passport;
vi. List of Properties (certificate/s of title, tax declarations), if any;

b. Consent of the following persons (in the form of affidavits);

i. Adoptee;
ii. Father of adoptee;
iii. Legitimate children of adopters who are ten (10) years of age or over;
iv. The illegitimate children of the adopter living with him who are ten (10)
years of age or over, if any;
