Research Questionnaire

Good day!

We are one of the groups from Grade 12-Honesty of A.O. Floirendo National High
School. We are currently conducting a research study entitled, “Level of Public
Speaking Anxiety in English among Grade 12 Students" as part of our Practical
Research 2 subject. The said study aims to investigate students’ perception with regard
to public speaking anxiety.

In this regard, we are humbly asking for your time and cooperation in accomplishing this
research questionnaire with all honesty. Your cooperation will be highly valued and
appreciated as we believe that it will lead us towards the realization of our study.

Rest assured that all information provided will be treated with utmost confidentiality and
will not be shared with anyone other than the researchers. The data that will be
