Depending on the application they are paper based or electronic based. Paper based: 1. Heading to make the purpose of the form clear. 2. Make clear to the person the place to fill in the answers. 3. Use textboxes to limit the amount of information. 4. Use character boxes for surnames, telephone numbers. 5. Use printed text boxes for dates 6. Use of tick boxes for selection answers 7. Make sure of sufficient place to write the answers 8. Make sure of clear fonts and text colours to ensure the form is easy to read. Computer based: 1. Use textboxes to capture key data 2. Use of on screen help when completing the form 3. Use of drop down / combo boxes where there are limited choices 4. Use radio buttons and tick boxes requiring single click of mouse to select 5. Automatic validation of data 6. Control buttons such as next form, clear entry, save 7. Double entry boxes to check correctness of key data.

Testing: need for testing 1. File structure should be finalized and make sure it is robust when system goes live. 2. Correct data is stored. Validation routines and verification methods are used to make sure this happens. These routines have to be tested and also make sure that data transferred from paper based to electronic system has been done accurately. 3. Hardwares used to interface with the final system has to be identified.
