Purpose
The purpose of the lifting plan is to identify the control measures necessary to negate the primary
hazards of:-
The crane collapsing or falling over
The load falling from the crane
The load or machine striking someone or any other identified hazard

Sequence of Operation
Note: List the Sequence of Operation in a way to do the task safely e.g. (Crane comes first or Trailer, Lift Load # 2 before Load # 1
…etc.).
Contingency Plan
Note1: Basically it is Plan B should things changed i.e. (Crane 1 break-down and Crane 2 can be used with some changes in the
Plan). Note2: Emergency Response Team availability at site or nearest Hospital / Clinic …etc.

site.

The following non-exhaustive list should be considered during the lift plan along with any other factors that could
affect any aspect of the lift:

Hazard Control

Strength and stability

Ground conditions must be suitable and sufficient and remain so during crane lifts to take the anticipated loads.
Include details of the ground conditions and any additional works required to the ground, including spreader mats
specification and any testing regime required (for example, CBR tests).
A temporary works design must be in place and checked off before any lifting operation or crane set-up takes
place.

Weather/environmental considerations

Indicate in this section detail of wind speeds/Sea state/ environmental conditions that have been
referenced/anticipated for the duration of the lifting operations and conditions that lifting operations must stop.

Access
Include travelling routes, road closures/highways notifications, and access problems.

Third party considerations

Over-sailing, works adjacent to railways, noise and restriction of view.

Emergency/breakdown procedures

Signaling

Indicate whether hand signals or radios are to be used. Also indicate signalling source and radio
frequency/channel. This section can also include any unique slinger/signaller identification (that is, different
colour helmet/hi-vis) if required.

Site and crane layout plan

Include crane position, lay-down/rigging area, position of delivery vehicles, landing points, banksman positions,
ground and overhead hazards and exclusion zones. If required use elevation drawings.

Safe system of work - methodology

Include pre-lift, lift and post-lift.

The details of this lift plan, along with any other associated risk assessment, method statement or safe system o
work, have been brought to the attention of, and explained to, the persons listed below, who have acknowledge
that they understand the contents, hazards and associated control measures.

Other relevant documentation (list and attach)

Complex Lift Plan File Note shall be available following Asset Integrity Lifting Engineer (Lifting Technical
Authority) or Site SPA (Single Point Authority) Approval prior to commence the actual Lifting Operation
(Refer to Appendix V).

In the event of any changes to circumstances, personnel or equipment, the plan should be reviewed and
revised by the Lift Planning Engineer (appointed person) and any changes recommunicated to the lift team.

Notes:

a. Routine lifts: Load Net Weight less than or equal to 80% SWL normal operating parameters of the lifting
equipment, permanently installed lifting equipment, certified lifting point or points, ample headroom to
complete lifting operations, not lifting over live plant or occupied areas or people, not lifting over or in close
proximity (less than 3 m) to active or energized hydrocarbon process or pressurized utilities process
equipment, load has known and uniformly distributed weight, uniform shape, identified Center of Gravity and
packaging robust, Center of Gravity below lifting points, lifts with pre-slung arrangement and/or not using
specialist lifting accessories, suitable lay down are is available, not lifting persons, single lifting appliance.
b. Non-Routine SIMPLE: Load Net Weight less than or equal to 80% SWL normal operating parameters of the
lifting equipment, temporary installed lifting equipment, certified lifting point or points, limited headroom to
complete lifting operations, not lifting over or in close proximity (less than 3m) to active or energized
hydrocarbon process or pressurized utilities process equipment, load has known and uniformly distributed
weight, uniform shape, identified Center of Gravity and packaging robust, center of gravity below the lifting
points, lifts without pre-slung arrangement, using specialist lifting accessories, attachments or slinging
methods (Non-standard rigging arrangements), suitable lay down area available, single lifting appliance, not
lifting persons, using proprietary lifting accessories or attachments for vehicle loading and unloading,
skidding operation using Skates & Ground Trolleys and cross-hauled the load or restrained using two or more
non powered lifting appliance with 100% redundancy.
c. Non-Routine COMPLICATED: Load Net Weight “without any Factor of Safety” more than 80% SWL but less
than 90% SWL with exception of Overload Testing of Fixed and Mobile Lifting Appliances, temporary installed
lifting equipment, non-certified lifting point or points, limited headroom to complete lifting operations, Lifting
in close proximity (less than 3m) to active or energized hydrocarbon process or pressurized utilities process
equipment but not over life plant / line, load is unknown or not uniformly distributed weight, or not uniform
shape, or difficult to identify Center of Gravity and not robust packaging, Center of Gravity above lifting
points, Use of special lifting accessories, attachments or slinging methods (Non-standard rigging
arrangements), unsuitable or unavailable laydown area, use of 2 or more lifting appliances including tailing
pipe using winch and crane, lifting persons by crane including drill floor Man-Riding winch operations, load
lowered into or lifted from a confined space, if wind load is calculated with applicable Crane wind speed
limitation deduction as per OEM, lifting & transferring loads from one offshore vessel to another and Overload
testing,
d. Non-Routine COMPLEX / CRITICAL: Recovery of a failed lifting operation, Load Net Weight “without any
Factor of Safety”=>90% of SWL; however Dynamic Lift Load which is compared to Load Chart shall not
exceed 100% SWL considering all applicable Factors of Safety, temporary installed lifting equipment, non-
certified lifting point or points, limited headroom to complete lifting operations, lifting over active or
energized hydrocarbon or pressurized utilities process equipment, load is unknown or not uniformly
distributed weight, or not uniform shape, or difficult to identify Center of Gravity and not robust packaging,
Center of Gravity above lifting points, large loads (surface area / volume) requiring specialist lifting and
slinging requirements, tandem lift with 2 cranes or more, dangerous goods cargo lifting such as explosives,
propane tanks etc., load is special and / or expensive the loss of which would have a serious impact on
production operations, snatching a load from untied vessel for non-DPII Vessel, transferring the load from
one lifting appliance to another, lifting with a helicopter and subsea lifting (All the load to be submerged in
the water regardless of Crane's hook block).
