MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This PARTNERSHIP AGREEMENT is entered into this 7th day of April, 2021 at Brgy. Mapulot,
Tagkawayan, Quezon by and between:

MAPULOT ELEMENTARY SCHOOL, located at Phase 1, Brgy. Mapulot, Tagkawayan,

Quezon, represented by the School Principal, EDELETH M.CORTEZ;

-and-

SANGGUNIANG BARANGAY, Brgy. Mapulot, Tagkawayan,Quezon, represented by the

Barangay Captain, ROGELIO S. ESMAS;

WITNESSETH:

WHEREAS, both parties recognize the need to broaden the access to education in this time of
pandemic through the Project SIKLAB (Silid-Aklatan sa Barangay) by maximizing the use of
facilities available in the Barangay.

WHEREAS, the School oversees that putting up a book hub (hereinafter called Silid-Aklatan sa
Barangay) will stimulate students’ acquisition of knowledge and develop the love for reading;

WHEREAS, the School sees that “Silid-Aklatan sa Barangay” will ultimately benefit not only the
students but also the community as a whole;

WHEREAS, the School has signified its commitment to provide supplementary materials that will
support and assist the need of the students in the Barangay;

WHEREAS, the School is spearheading the implementation of “Silid-Aklatan sa Barangay” in the

Barangay.

WHEREAS, the Barangay sees that the “Silid-Aklatan sa Barangay” can be of great help to students
dilemma in the new normal curriculum implementation thus agreed to lend one vacant facility to
be used;

WHEREAS, the Barangay is willing and committs to support and assist the School with its project in
order for this to succeed;
NOW THEREFORE, above premises considered, the Parties hereby commit themselves to perform
their respective obligations under this Agreement:

IN WITNESS WHEREOF, the Parties have caused this Agreement to be signed in their names on the

 MEMORANDUM OF AGREEMENT
This contract of agreement is made and entered into in the City of Lucena, by and between:
The MANUEL S. ENVERGA UNIVERSITY FOUNDATION, a non-stock, non-profit
educational institution, existing under the laws of the Philippines with principal address at Brgy.
Ibabang Dupay, Lucena City represented herein by _____________, President and Chief Operating
Officer, hereinafter referred to as MSEUF.
The _________________________, a public institution existing under and by virtue of the
Laws of the Philippines with principal address at ________________ being represented herein by
_______________, Principal, hereinafter referred to as ______.
Witnessed that:
WHEREAS, the University Libraries of Manuel S. Enverga University Foundation accept
the responsibility of being an instrument in the realization of the University mission not only in
educational aspects but also along the line of community service;
WHEREAS, the University Libraries will provide reading materials, that will support the
objectives of the University’s mission and add strengths to the educational, physical, social, and
spiritual progress of ______________ as the major beneficiaries of the program;
WHEREAS, the MSEUF with its commitment to the service of God and country is willing
to undertake community service programs that will serve ________________________ who should
have an equal chance in improving their lives through education, livelihood skills and socio-cultural
development which can be their tools to be more effective and useful citizens.
WHEREAS, the _________________ aims to give equal opportunity to all its students and
faculty by doing its best in looking for different agencies and institutions that can help address its
needs;
NOW THEREFORE, for and in consideration of the foregoing premises, the parties hereto
bind themselves to this Agreement, subject to the following terms and conditions, to with;
1.0 MSEUF shall:

1.1. Coordinate with ____ regarding availability of books, journals, magazines and other
supplementary materials for donation to the latter, which beneficial for the instructional
needs of the school and reading/research needs of the personnel of ______ via online.
1.2.Assist and train _______ library personnel specifically in the organization of the
materials through cataloging and classification via online
1.3.Provide recommendations via online in the planning and supervising of the library, its
facilities and collections.

2.0 MCES Learning Resource Center shall

2.1. Identify specific needs and coordinate with the ________ via online relative to the
library materials which will be picked up from the library;
2.2. Provide necessary online assistance for the implementation and continuance of the
program;
2.3. Assist whenever needed the volunteer workers and staff in the conduct of community
service activities online;

NOTE:
This is just the draft of the Memorandum of Agreement
signed by the representatives of the school and the
university to know the content of the said
memorandum.
The original MOA is still in the University to be
notarized.
