DEED OF DONATION

WITH ACCEPTANCE OF DONATION

This Deed of Donation is made and executed by and between:

LOCAL GOVERNMENT UNIT OF SAN JOSE , a government unit structured and

existing under and by virtue of the laws of the Republic of the Philippines, with principal address
at Villa Aglipay, San Jose, Tarlac represented herein by its Mayor ROMEO G. CAPITULO and
hereinafter referred to as the “DONOR”.

DEPARTMENT OF EDUCATION-SCHOOLS DIVISION OF TARLAC

PROVINCE DIVISION, a component of the Department of Education with office address at
Macabulos Drive, San Roque, Tarlc City, Tarlac represented herein by Head Teacher-III Rodel
B. Valdez and herein referred to as the “DONEE”

WITNESSTH:

WHEREAS, the DONOR desires to participate in the program about Modular Distance
Learning by Division of Tarlac Province.

The DONOR therefore hereby DONATES, TRANSFERS and CONVEYS unto the
DONEE Radio Equipment needs for Installation of Radio Station free from all liens and
encumbrances.

That the DONEE, by signing the conformity portion therefore hereby accepts the
donation of Radio equipment valued at Sixteen thousand seven hundred twenty and hereby
expresses its gratitude to the DONOR.

IN WITNESS WHEREOF, the parties, through their respective representatives, have

signed this Deed at the Sta. Cruz Elementary School, Sta. Cruz, Iba, San Jose, Tarlac on
February 26, 2021.
