    SAFETY INDUCTION FORM

    Welcome visitor......

    Your safety and pleasant visit is our first priority. So please spend a few minute checking whether you understand our
    safety procedure. Please tick the following items when applicable;

    I was provided with proper PPE that suited my visiting purpose.

    I know where the Muster Point Area is.

    I know the Main Escape Route.

    I know the Alternative Escape Route.

    I should stay within the safe walk way which painted with green unless suggested otherwise.

    I know the area where smoking is allowed and where it is prohibited.

    I understand the procedure during the event of emergency.

    During an emergency situation, please proceed immediately to the Muster Point Area. Do not attempt to put off any
    fire or involved yourself in dangerous situation. Our emergency response team will handle the situation.

    I had received a safety induction by SDI personnel and fully understand the content.
