As per above referred request, Audit lifting equipment’s of PT. SCHLUMBERGER CCU 17-Unit has been performed at
Bekapai – BJ Platform area (AWB FIORE 270i) on October 29, 2022 with scope of audit as follow:
1. Document review
2. Physical verification
3. Function Test

Audit Findings
1. None

Conclusion & Recommendation

Based on re-audit result, the Lifting Equipment’s (CCU 17-Unit) are Acceptable to be used in PERTAMINA Hulu
Mahakam premises, with recommendation as follows:
1. None

This MEMO is valid until April 28, 2023.

Please be re-certificated before in due date.
If not re-certification in due date, this memo is considered not valid any more.
Refer to Permen ESDM No.32 Years 2021, the inspection company must have latter approval issued by Dirjen Migas.
If not available letter approval by Dirjen Migas, we considered Not Valid the result inspection of inspection company.

Note :
 Refer to PERMEN ESDM No. 18 Years 2018, in article 8 mention the inspection company must have SNI ISO/IEC 17020 and
PERMEN ESDN No.32 Years 2021 in article 14 mention the inspection company must have latter approval by Dirjen Migas.
 User responsible to make sure all certificate lifting gears are valid for purpose.
 There is any change to lifting gears or CCU condition (major or critical component) after this audit the LE subject to re-audit.
 A solid color code rectangular YELLOW (RAL-1007) is to be applied to the lifting equipment which Acceptable to be used,
and rectangular RED – (RAL 3020) is to be applied to lifting equipment which Not Acceptable.
 The Inspection CCU or offshore container (Visual, NDT, Load Test, etc) refer to BS 7072
