Is the working over or near water is planned and is part of adequate method
statement with suitable risk assessment?
Are all workers working over the water, and in areas on vessels that are
unprotected by permanent barricading wearing personal floatation devices?
Has the Contractor provided life rings, fitted with 30m of life-line at regular
intervals (50m) along the work face when working near the water.
Do access walkways, jetties, quay walls, mooring locations, temporary barges
and crew boats have signage indicating the requirement to wear PFDs when
working?
Is the persons working near or on board are trained for sea survival and has
relevant certification?
Is the hazard and associated risk has been made aware to the people working
near or over the water and needed controls have been briefed through toolbox
before start of work every day?
Are all hydraulic, lubrication, fuel hoses and storage areas in good order, free
from abrasion and unions tight and free from leaks?
Is the adequate and competent supervisor is present to monitor the work
throughout execution?
Are walkways on vessels clearly laid out and demarcated to keep workers
away from hazardous areas. Are these walkways free from stored material and
treated to ensure they are not slippery when wet?
Are all vessel mounted cranes, derricks and lifting devices fitted with working
operating alarms?
Are all vessels carrying sufficient fire extinguishers or fire fighting devices that
are correctly labelled and regularly inspected?
Is worksite lighting adequate for the work being undertaken ensuring that no
work is being undertaken in shadows?
Are flammable and hazardous materials including compressed gas cylinders
stored in a safe manner, with secondary containment where appropriate (i.e.
for fuels and oils)?
is adequate emergency preparedness is there to rescue the person over board
and for recovering the persons from life rafts
