A. This form is to be completed by Owner/Operators of tanks that store liquid organic

material. The data is the inputs required for the USEPA entitled "TANKS 4.09b". The
web address for this software is as follows:

B. Start with the spreadsheet entitled "Comp_Fac_Cer". Next complete a spreadsheet that
corresponds to each type of liquid storage tank at this facility. The type of storage
tanks are as follows:
Horz_Fix = Horizontal - Fixed Roof
Ver_Fix = Vertical - Fixed Roof
Int_Flo = Internal Floating Roof
Ext_Flo = External Floating Roof
Doom_Flo = Doomed External Floating Roof
C. Only place data in Aqua colored fields.
D. The color code for the colored fields is as follows:
