SUMMARY OF DEVELOPMENTAL ASSESSMENT

Name: SAPINOSO, Jairic Eliezer Date Evaluated: 09/12/2023

Birthday/ Age: 05/20/2020 (3 years and 3 months)

Pertinent History:
Jairic Eliezer “Eli” is a 3-year-old male who consulted due to concerns in behavior and communication. He is not responding to his
name when called and has no eye contact. Speech delay was also noted; would express his needs by hand-leading. He is hyperactive
and is prone to meltdowns. Repetitive behaviors during play like lining up and making things fall and stand up (e.g., electric fan).

During the assessment, Eli had fair-good eye contact. He speaks in 1-word phrases but mostly would jargon. He answers “what”
questions when looking at pictures but would not answer simple questions like “what’s your name” or “how old are you”. He also
inconsistently responds to commands; would need several prompts. He gets distracted with toys and would not focus on the
examiner while playing. At times, he would bang toys repeatedly.
