Greetings!

The Siruma Municipal Hospital under the Provincial Government of Camarines Sur is engaged in providing basic
health care services which generates substantial amount of infectious and pathological wastes.

To free our limited storage and to protect our health care workers as well as the general public on possible
exposure, it is necessary that these materials be transported to the Treatment Storage and Disposal (TSD)
Facility for treatment and final disposal at the earliest possible time. In this connection, we would like to request
from your good office the issuance of a one-time Special Permit to Transport (SPTT) while our online generator
ID is still in process as we are complying the requested documents.

As such, we humbly request for the issuance of the SPTT for M501 with the quantity of 1.096 metric tons to be
transported by ENVIROCARE MGT. PRECISION, INC, as our accredited transporter with Transporter Registration
No. OL-TP-R3-14-000143. The same shall be brought to the facility of MAYA MED WEST CORPORATION located
at M. Villarica RD. Prenza 1, Marilao, Bulacan with DENR ID OL-TR-R3-14-000565.

Attached to this request are the pertinent documents and other requisites for review.
