Dear sir,
With respect to state that, for my father’s emergency lung cancer treatment
in Saint Louis Hospital, Bangkok, Thailand through Malaysia, I must need to
take leave for one week starting from 04/07/2023 to 10/07/2023. I would
like to inform you that my father’s lung cancer surgery was completed on
18/03/2023, but the doctors detected the spread of cancer in lung again. So,
we need to take him for farther treatment. The medical documents are
attached with this application for your information.

I have taken the classes and assignments to retreat the situation in advance,
about which the department is well informed.

I, therefore, hope that you will be kind enough to grant my application and
oblige thereby.
