INQUIRY REPORT AGAINST THE MATTER OF MISAPPROPRIATION OF SAFETY


HAZARD MATERIAL IN THE O/O SDO IESCO SUBDIVSION DINA-II.

XEN (E) IESCO Operation Division No.II Jhelum constituted an Inquiry


Committee comprising of following Officers / officials vide Office Order No.134/25444-49
dated 12.09.22 to probe into the facts & figures regarding installation of 02 number of poles
at different locations IRO DINA-II Sub Division and fixing the responsibilities to proceed
further.

TASK OF THE INQUIRY

To probe into the Facts & Figures mentioned in the Consumer complaint
lodged by Mr. Raja Waseem and checking the documentation of the said pole cases as
well.
PROCEEDINGS

The date of Inquiry was fixed on 26.12.2023 in the office of SDO IESCO Sub
Division Dina-II. The undersigned summoned the individual involved in this case. I being the
Convener of Inquiry Committee along with Mr. Ameer Muavia SDO IESCO Operation Sub
Division Dina City and Mr. Umair Ahmed SDO IESCO Operation Sub Division G-9 for
Inquiry proceedings. The individual involved in the inquiry case was questioned in respect of
different aspects of consumer complaint. Mr. Muhammad Rizwan LS-I was quizzed
regarding documentation related to number of illegal poles installed at Noshahi Hotel and
Kotli Behram respectively as claimed by complainant. To the question of feeder incharge,
Mr. Rizwan LS-I said that due to suspension of then feeder incharge Mr. Khan Badshah LS-
II, the SDO instructed him to look after the feeders until further orders. The LS Mr. Rizwan
stated that one number of dismantled HT pole was installed near Noshahi Hotel under as
per the instructions of SDO. It was claimed by him that area lineman also indicated the poor
and rusty condition of old LT pole, which resulted in tilting of pole and creation of loose sag
in LT span leading to frequent outage of supply in the vicinity of said area. Mr. Rizwan LS-I
also declared that LT pole was replaced with dismantled HT pole on the issuance of SJO by
concerned SDO with the instructions to restore the electricity of said area. On question
regarding installation of pole in Kotli Behram, Mr. Rizwan LS-I claimed that he did not install
the pole in said area. However, he stated that a SDG work was done by the ELR
subdivision of Construction formation of Jhelum Division and poles were installed in said
village. On further investigation, it was determined that the LT pole was installed in the Kotli
Behram village under the category of approved works which cannot be claimed illegal.
FINDINGS / CONCLUSIONS.

Keeping in view of above facts it is observed that poles installed at the


respective locations in the jurisdiction of Dina-II subdivision are not with any violations of
IESCO SOPs. The dismantled HT pole installed at Noshahi Hotel was found to be
acceptable as per site position and departmental procedures. The LT pole case of Kotli
Behram village was not executed by the said individual as claimed by complainant. It was
found that the SDG work was executed by construction formation and the LT pole was
installed under sanctioned works. From the proceedings and findings of the inquiry
committee it is deduced that poles installed at different locations of Dina-II subdivision are
legit ones. Therefore, it is concluded that Mr. Rizwan LS-I did not take part any illegal
installation of poles and the material is installed at these sites after completion of
departmental proceedings.
