II DESCRIPTION OF THE INCIDENT:


While Mr. A. Pascua was prying the hang up boulder above the arcgate, suddenly the
boulder went on rush hitting his pry bar causing it to be hitch against the concrete back and
pinning his right middle and ring fingers.

III. FINDINGS AND OBSERVATIONS:

1. Mr. Pascua was assigned at loading 15/16 of conveyor 20.


2. He noticed that the water pressure was low so he proceeded to Loading 17/18 to
close the water valve thereat.
3. At Loading 17/18, he observed that loading 18 was on hang up status so he decided
to pry underneath the arcgate.
4. On his 1st attempt, it was successful and goes hang up for the second time after pull-
out.
5. Engr. Joel Legaspi tried to pry twice but to no avail.
6. Mr. Pascua change Engr. Legaspi to do the prying. This time, he pryed above the
arcgate as he sees an opening above the arcgate.

7. Upon prying above the arcgate, suddenly the hang up fell and hit his pry bar where it
was hitch at the concrete back pinning his right hand 3rd and 4th digit fingers.
8. He was able to release his hand while the pry bar fell and was recovered by Engr.
Legaspi.
9. Other IPIC findings:
a. There was a wide gap (about 6”) above the arcgate due to worn out bumper.
b. The deep of the hang up boulder was about 7-8 “ from the tip of the arcgate
which could be easily be seen prompting him to pry it on the wide gap.
c. He uses a prybar with about 6’ X 1” diameter.
d. He was an old timer chute tapper assigned at the CHC Loading area.
e. He knows the SJP that prying above the arcgate is strictly prohibited due to
the risk involve but he was tempted to do it above the arcgate as he was
tempted and is easier to bring down the hang up boulder.

IV. CAUSE ANALYSIS:


Immediate Cause: Unsafe act – Improper placement.
Mr. Pascua should have done the prying below the arcgate as the standard
procedure and not above the arcgate taking advantage of the worn out bumper.
Basic Cause: Personal Factor – Poor Judgement.
Mr. Pascua, despite knowing the hazards of prying above the arcgate, he did
shortcut to pry at the open gap cause by the detached bumper.
