1.0 INTRODUCTION
1.1 Project Overview
Jangkrik field is located about 70 km from the coast, in the Muara Bakau permit area,
Makassar Strait, offshore Kalimantan, at water depth ranging from 250 to 500 m. ENI
through its Indonesian subsidiary, operates the Muara Bakau PSC.
The treated gas and condensate from FPU are exported via 24 inch treated gas export
pipeline and 4 inch condensate export pipeline to shore at Sapi landfall. An onshore
receiving facility (ORF) fiscal metering system for gas and condensate is located
approximately 4 km downstream of Sapi Landfall. Gas and condensate pipelines are the
tie-in to existing East Kalimantan pipeline network. Another tie-in at KM 21.65 is required
for the gas line to allow the full capacity of jangkrik gas. For this purpose, a 15 km of new
pipelines is to be installed parallel to existing line with PHM ROW from ORF to KM 21.65,
and will be tie-in to existing 20 inch Handil-Nilam pipeline and modification to be done at
Nilam, BEM, and KM 53 inside POMA area. Final destination of Jangkrik treated gas is to
the Bontang LNG Plant and domestic Bontang market; and the final destination of Jangkrik
condensate is to Senipah Plant.

1.2 Document Scope


The scope of this document is to define the CONTRACTOR procedure on performing the
demolition activities at PK52 Nilam, for the awarded project called “ EKS Modification at BEM
and KM 53 Area“. This activities describes the risk assesment, preparation and execution.

The purpose of this document is to provide the detail method and sequence for demolition
work PK52 Nilam at EKS Modification at BEM and KM 53 Area Project. This procedure define
the demolition such as ground back filing, compacting, scaffolding, bolting unbolting, and
lifting activities.

4.0 ROLES AND RESPONSIBILITIES


4.1 Site Manager
He is responsible to manage yard facilities and utilities as required or as per project condition and
approved procedures. He has to communicate with COMPANY site representative about related
issue of the work. Prepare, support and maintain requirement of all tie-in activities. Coordinate with
client and in-house related field counter parts.

4.2 Superintendent
He is responsible for all activities at yard, preparation tools and equipment, prepare tie-in materials
including installation works as per approved procedure, COMPANY specification, HSE requirement
and ensure all works are in line with work plan.

4.3 Piping Supervisor


Supervisor shall be responsible for overall tie-in activity. He shall be responsible for the works and
will oversee all construction work as per approved method statement, COMPANY specification and
HSE requirement.

4.4 Lifting Supervisor


Lifting supervisor shall be responsible for lifting operation of materials or equipment as per
approved procedure, COMPANY specifications and HSSE requirements. He shall also be
responsible for checking the lifting arrangement and crane working radius during the lifting
operation..

4.5 Quality Control Inspector


The QC Inspector shall be responsible for daily in process inspection for fit up activity, bolting
unbolting activity, conduct joint inspection with COMPANY representatives and prepare all required
inspection documents.

4.6 Safety Officer


The safety officer shall be responsible for the implementation of the HSSE plan and compliance
with all statutory regulation.

4.7 Lifting Authority


Providing technical overview for lifting operation on all sites with related party.

4.8 Slinger
Participate in the planning and TRA of lifting operation and pre-lift safety meeting. Prepare the
load for lifting according to LOP and complete pre-use inspection of liftng accessories before use.

4.9 Rigging Loft Controller


Shall issue equipment, accept returned equipment, maintain the loft registers, inspect equipment
on receipt, inspect equipment prior to issue and inspect equipment on return.

4.10 Rigger
Riggers are responsible for:
o Attaching, detaching slings to and from the load.
o Riggers are appointed where it is necessary to ensure that loads are correctly attached to
the lifting equipment.
o Rigger also monitoring/ control the risk due to equipment movement and guide/ inform to
lifting superintendent/ safety man.

4.11 Banksman / Signaller


Signalman are appointed where it is necessary to ensure that clear and precise commands are
given to the operator of the lifting equipment, either in carrying out the lifting operation or when the
equipment is being move. During lifting operation, Signalman can not to touch the load.
Signalman is responsible for:
o Relaying signals to the equipment operator and for initiation and direction of safe movement
of the lifting equipment and hook load.
o Directing the safe movement of the equipment and its load to maintain the safety of all
personnel, Attaching, detaching slings to and from the load.
o Communication device (radio) shall be used during lifting operation.

4.12 Foreman
Shall be responsible for overall demolition activity site. He shall be responsible for the works and
will oversee all demolition work as per approved method statement, COMPANY specification and
HSE requirement.

4.13 AGT
 The AGT’s are approved persons who have been formally trained (Certified) and
authorized in gas testing techniques and methods.
 Authorized to test for the presence of flammable vapors, toxic gas and oxygen as
requested by the Performing Authority (PA).
 Competent to carry out gas testing on all activities including testing for explosive
atmospheres and oxygen deficiency.
 Ensure the work site in a safe condition report the result of his tasks direct to the
Performing Authority.
 Continue monitor the atmosphere whilst personnel are within the confined space
entry.
 Keep entry log up to date at all time with CSE permit its must be keeped with and
returned with the PTW
 Level 3 (CTR) Competent to monitor continuously using a personal gas detector

4.14 Pipe Fitter


To perform demolition work activity as per approved procedure.

4.15 Helper
To assist fitter and support demolition activity.

4.16 Scaffolder
 Assure that all scaffolding they will be working on has been installed and inspected
and is safe for the employee to work on, before beginning work.

 Wear all appropriate PPE and an approved harness with shock absorbing lanyards
with double locking snap hooks will be worn at all times while working off of a scaffold.
 He is responsible for safety of the scaffolding teams during erection, dismantling
operations including selection of PPE and protection of working area.
 Finding out details of work to be carried out for the scaffold and to ensure that it is
properly constructed and of the correct type for the requested duty.
 Shall have scaffolding training and certified by third party.
 Shall have working at height training and certified.
 Scaffolder shall be pass assessment by COMPANY.

4.17 Scaffolder Inspector


 The scaffolding inspector will be act as Performing Authority (PA).
 Scaffolding inspector is fully engaged during developing scaffolding mapping.
 Engage in risk assessment and task specific ERP.
 Own and passes scaffolding training and certified by third part and COMPANY
assesment.

4.18 Boom Truck Operator


Shall be responsible to operating Boom Truck as per Company regulation.

4.19 Lifting Appliance Operator


Participating in the planning, including TRA and pre-lift safety safety meeting of each operation.

4.20 Crane Operator


o Correctly operating the equipment in accordance with the manufacturer’s instructions and
the safe system of work.
o At any time responding to the signaler’s signal.
o Performing the pre-use inspection.
o Filling up the equipment log book and recording any mechanical, or structural, defect and
any accident, or unusual occurrence, all of which must be reported to the relevant
superintendent.
o Ensuring possession of valid certificates.
o Ensuring communication with rigger/ signaler.

4.21 Transport Coordinator


 Arrange schedule for driver on duty and rotation system
 Shall ensure all driver to obey the driving rules and drive safely
 Shall ensure all driver to perform daily vehicle checklist
 Record all vehicle kilometer driven

4.22 Vehicle Driver


 Driver shall pass DDC and attend HSSE Induction.
 Carry out daily / weekly / monthly checks using pre-use or periodic inspection checklist.
 Working hours shall not exceed 16 hours / day and shall have off day min. 1 day in a week.
 Driver shall hold a valid driving license for the type of vehicles they are operating.
 Driver shall obey company-driving rules and respect with others warning signs posted as
per road safety rules and regulations at the area he operates.

 Driver shall ensure that he and his passenger are fastened their seat belt prior driving
vehicle.
 Driver must turn off the engine while refueling.
 Driver shall perform a daily inspection checklist of their vehicle as per attached formats and
reports submitted to transport coordinator on daily basis.
 All vehicles incidents involving COMPANY owned / leased vehicle on company business
shall be reported to transport coordinator immediately.
 Driver shall assure that STNK & SIM vehicle license is valid.
 Driver shall have a valid MCU.
 Driver shall assure that his vehicle is maintained and ready.
 Driver shall not use mobile phone, under alcoholic or additive substances while driving.
Driver shall have awareness to stop unsafe action or condition.

5.0 SAFETY
5.1 Coordination Meeting and Tool Box Talk (TBT)
To ensure the safe execution, CONTRACTOR shall perform daily Tool Box Talk (TBT) in every
beginning working shift before the work started. In this TBT the Construction Lead/ Supervisor will
explain the work which will be done during the working shift, explain the HSSE risk on every
working step, and the mitigation to eliminate/minimize the risk to all of the working team.

Daily coordination meeting between COMPANY and CONTRACTOR will be done to discuss on the
elapsed day daily progress report focusing on the construction concern and the solution plan.

Weekly progress meeting will be done in weekly basis, discussing on the elapsed week progress
report, focusing on the HSSE & construction concern and the solution plan, next 2 weeks look
ahead schedule, and recovery plan (if any).

5.2 General
To minimize the risk of accidents during the work, safety precautions and risk assessment has
been prepared as guidelines for the all personnel involved. Safety induction shall be organized
upon mobilization of all personnel on the site, including COMPANY personnel.

5.3 Personal Protective Equipment


All personnel will be provided with Personal Protective Equipment (PPE) that related to the work to
be performed, i.e. safety shoes, safety helmet, safety glass, hand gloved, ear plug, including
specific PPE such as body harness, working vest, etc. and shall be used at all times during the
works.

5.4 Work permit


No work can be carried out without any valid work permits. The works shall not start before the
work permit is opened. CONTRACTOR prepares the work permits at least 24 hours in advance. All
work permits shall comply with PHSS work permit procedure for any intervention inside POMA and
PHSS premises, and CONTRACTOR Work Permit Procedure for the outside works. Welding
inside hazardous area is classified as hot work, and shall be covered with hot work permit

5.5 Emergency Response Plan (ERP)


ERP has to be communicated to all the team during tool box talk’s prior start the job, Safety talk
and Tool Box Talk shall be share the hazard, mitigations and brief clearly too all personnel’s what
to do during emergency.

5.6 Muster Point (MP)


The muster point location shall be a free area that easy to be accessed.

5.7 Environment
The installation activities should minimized impact to the environment. Any waste material and or
unused material shall be discharged to designated area that approved by Government bodies

7.0 DEMOLITION WORKS


7.1 Scope of Work
The scope of Demolition Works refer to “NLM-M-10A-022A” is consist of :
1. SDV 4852 Relocation.
2. Ball Valve 20” 600#, 2 Ea and 4” Piping Line Removal.
3. Nitrogen Tank and Instumentation Line Removal.

Demolition work shall be conducted in struct accordance with approved procedure and LOP. Any
variation from the agreed procedure and LOP shall result in the job being stopped and reassessed to
ensure continued safe operation.

7.2 SDV Relocation Sequence


1. Pre-Job Meeting Shall be done and attended by all parties.
2. Make sure crane and rigging arrangement follow latest document.
3. Position crane as per LOP. (See Appendix C for details)
4. Instal Rigging Gear to SDV 4852. (See Appendix C for details)
5. Lift up SDV 4852 for 2M to give safe clearance between SDV 4852 and existing piping.
6. Swing the crane’s boom to the left to destined laydown area (truck).
7. Install blind flanges to opened line.

7.3 Ball Valve 20” 600#, 2EA and 4” Piping Line Removal Sequence
4” Piping Line Removal
1. Remove all valves and equipment that connected to demolished line.

3. Install Blind Flanges to opened line.

Ball Valve 20” 600#, 2 EA Removal

4. Position crane as per LOP. (See Appendix C for details)


5. Install rigging gears to ball valve 20” (1). (See Appendix C for details)
6. Attached webbing sling 5T that connected to Ball Valve 20” (1) to the crane’s hook.
7. Lift up the hook.
8. Unbolting Ball Valve 20” (1).
9. Pull out Ball Valve 20” (1) from initial position using chain block 2T.
10. Once Ball Valve 20” (1), Guide Ball Valve 20” (1) to open area.
11. Install Blind Flange 20” 600# to opened line.
12. Lift up Ball Valve 20” (1) for 6.5M to give safe clearance between Ball Valve 20” (1) and existing
piping.
13. Swing the crane’s boom to the right to desined laydown area (truck).
14. Move crane to Ball Valve 20” (2) removal position as per LOP (See Appendix C for details)
15. Repeat Step 4 to 12.
16. Swing the crane’s boom to the left to desined laydown area (truck).

7.4 Nitrogen Tank and Piping Line Removal Sequence


Piping Line Removal
1. Remove all valves and equipment that connected to demolished line.
2. Remove remaining pipes.
3. Install Blind Flanges to opened line.

Nitrogen Tank Removal


1. Pre-Job Meeting Shall be done and attended by all parties.
2. Make sure crane and rigging arrangement follow latest document.
3. Position crane as per LOP. (See Appendix C for details)
4. Instal Rigging Gear to Nitrogen Tank. (See Appendix C for details)
5. Lift up Nitrogen Tank for 2M to give safe clearance between Nitrogen Tank and existing piping.
6. Swing the crane’s boom to the left to destined laydown area (truck).
