I, Leodin Roch S. Jayme, residing at Purok Bagong Lipunan, Buhangin, Davao
City, Davao del Sur, hereby authorize Leanne Therese S. Jayme, residing at
Dumoy, Davao City, Davao del Sur, to act on my behalf regarding the LBC
shipment with tracking number 28390001161007. This authorization is valid
from February 5, 2024 to February 7, 2024 or until the specified task is
completed, whichever comes first.
I grant Leanne Therese S. Jayme the following permissions and
responsibilities regarding the aforementioned LBC shipment:
1. To receive the shipment on my behalf, including signing for it and
confirming its contents.
2. To pay any fees or charges related to the shipment, if required.
3. To handle any customs or clearance matters necessary for the release or
delivery of the shipment.
I understand that by granting this authorization, I am entrusting Leanne
Therese S. Jayme with the responsibility of managing this shipment, and I
release LBC from any liability or responsibility related to the actions taken by
Leanne Therese S. Jayme on my behalf.
Please consider this letter as a formal and legally binding authorization.
Leanne Therese S. Jayme will provide appropriate identification and proof of
this authorization when interacting with LBC representatives.
