WITNESS STATEMENT

Description of activity undertaken (please be as specific as possible)
Task 2 Participate in a work placement

For this task you will actually participate in a work experience placement. You will need to
provide evidence that you demonstrated you were able to:

Arrive on time
Follow instructions
Complete a given task
Respond to feedback appropriately

All participated in a placement at Bradley Woods Activity Centre where they worked
alongside staff to undertake tasks in the kitchen to produce lunch for the staff and
students attending that day. The placement required the students to arrive on time, wear
suitable clothing for working in the kitchen including apron and hair net, complete a range
of culinary related tasks.They served the food to staff/students. Ensuring that lunch was
ready on time to be served.

The tasks involved working as a team, following instructions, following rules and
procedures, staying motivated on tasks, communication, dealing with frustration,
decision-making and responding to feedback.

Erin prepared chocolate brownies for the class, she worked very well as part of the team,
helping others when asked. Good communication skills were again used in a positive
way. The kitchen this week was a little better equipped so tasks were easier to complete.
Erin again used the electric whisk and her confidence is growing in using this.

Harry set up the dining room as requested then made hot and cold drinks for service.
Harry was again an important part of the team, assisting others by cleaning areas,
sweeping floors and wiping up any spills which followed Health and Safety rules.

Ryan helped to make the vegetable rice, taking care to ensure the dish could be served to
vegetarians. Ryan worked well as part of the team, helping others when asked and
putting lots of effort into his dish, he was very good at organising the clean up of the
kitchen when service complete. Ryan followed any instruction given in a positive way.

Eugene followed instructions, worked as part of the team and helped anyone when asked
to do so. Eugene helped to make the mince beef filling for the enchiladas then helped
serve the meal. A great effort.

During the (Minimum of) 6 days on placement Noah has demonstrated a good work ethic
in all tasks. He has met the requirements of the criteria by arriving on time for every
placement day, demonstrating that he listens to instructions by completing tasks correctly
where instructions have been given to him verbally by the site manager. Noah has also
responded appropriately to feedback eg when he was using a pick-axe during a task to
break up hard ground Noah was stopped by the site Manager who reminded him how to
use it safely. Noah responded by adjusting how he was using it and asking “Is that
right?” He then thanked the site manager. Noah has completed all the tasks as detailed
in the section above

All three students arrived on time for their work placement day. They all listened to the
instructions from staff so that each of them were aware of what tasks to do for that
morning.
