Reading Intervention Plan Template

2. Objective:
The primary goal of this reading intervention plan is to help Lowell Albert L. Arenas increase his
motivation and love of reading and improve his fluency, vocabulary and comprehension skills in
order to meet or exceed grade-level expectations.

3. Assessment and Data Analysis:


a. Frustration reading level in Phil-IRI assessment result
b. Struggle in fluency, vocabulary and comprehension skills
c. Can read text but lacks enthusiasm

4. Goals and Targets:


a. Long-Term Goal:
To be able to read at or above grade level by the end of the school year
b. Short-Term Objectives:
Objective 1: Increase his motivation and love of reading
Timeframe: November 2023 to January 2024
Criteria: The student will be able to enjoy reading stories from various references.
Objective 2: Improve fluency, vocabulary and comprehension skills
Timeframe: November 2023 to May 2024
Criteria: The student will be able to read a grade-appropriate passage fluently and use learned
vocabularies for answering correctly comprehension questions.
Others:
Improve fluency by increasing words per minute from below 140 to above 140.
Increase comprehension skills by achieving 90-100% score on weekly comprehension assessments.
Expand vocabulary by mastering 25 new words per week.

5. Intervention Strategies:
a. Individualized Instruction: Provide one-on-one reading sessions with the pupil.
e. Fluency Practice: Use repeated readings, timed readings, and other strategies.
c. Comprehension Strategies: Teach reading comprehension strategies such as predicting,
summarizing, and questioning.
d. Vocabulary Development: Implement vocabulary-building activities, including word walls and
context clues.
f. Assistive Technology: Consider using technology and reading apps.

6. Materials and Resources:


a. Levelled Books: Phil IRI Selections, Stories from Learners Materials and Developing Reading
Power
b. Reading Worksheets: Create or use reading worksheets for practice and assessment.
c. Interactive Activities: Incorporate interactive reading games and activities.
d. Library Access: Encourage the student to read books of their choice for pleasure.
e. Digital Tools: Use educational software or online resources to reinforce reading skills.
