MEMORANDUM OF AGREEMENT

KNOW ALL MEN THESE PRESENTS:

This Memorandum of Agreement (MOA) made and entered into this ____ day of _______, 20 in __________

FIRST PARTY;

The Complete Address , represented by its LGU Mayor or dully Authorized

Representative ,___________________, hereinafter referred to as SECOND PARTY;

I. PROJECT
WHEREAS. The FIRST PARTY is offering Mobile Training Program in Qualification____, wherein
qualified learners undergo training in order to adequately train the learners to match industry needs and demands for
the future employment.
WHEREAS, the FIRST PARTY will provide all the Tools, Equipment, Materials and Consumables needed
during the training;
WHEREAS, the SECOND PARTY, agrees to provide training venue to be used for the whole duration of
training period.
WHEREAS, the SECOND PARTY, agrees to help the safe keeping of the tools, equipment and materials that
the first party provided for the training;
WHEREAS, both parties have mutual awareness and cooperation to be able to finished the training program
on time.
WHEREAS, the SECOND PARTY will scout the qualified beneficiaries / scholars.

NOW, THEREFORE, for and in consideration of the above premises, the parties hereunto have jointly agreed
to implement Mobile Training Program designed to achieve their common objectives as follows;

A. Name of School / FIRST PARTY shall;

1. Implementing and monitoring the Mobile Training Program in accordance on TESDA Training
Regulations.
2. Provide trainees basic skills with theoretical, general education values and human formation;
3. Coordinate to TESDA all the data needed by the agency for future assessment after the training.
4. Conduct mandatory assessment to all the trainees at the end of training if applicable.
5. Ensure that the provisions of this agreement are properly implemented and coordinated in the Second
Party for the purpose of ensuring high standard of training.
B. ______________________ /SECOND PARTY shall:
1. Assign coordinator to assist the trainer in monitoring daily attendance of the trainee and help the
trainer to motivate the trainees in completing the training program and undergo National Assessment
after the training.
2. Designate trainees conducive work place and environment friendly during the training duration.
3. Designate safe place to keep the tools, equipment and materials when not in use.
4. Ensure that the provisions of this agreement are properly implemented and coordinated with the First
Party.
5. Provide medical assistance to the trainee when injured during training.
D. EFFECTIVITY
The effectivity of this agreement will be based on the training duration stated below;

 It is mutually agreed upon that this agreement will not be modified except by written amendment
executed by the contracting parties.
