AFFIDAVIT OF ONENESS

KNOW ALL MEN BY THESE PRESENTS:

I, FARHAN BARSI ABDULGAFFAR, married, of legal age, Filipino

citizen and with residential address at 00217 Barangay Bubong, Lilod Madaya,
after having been duly sworn to in accordance with law, do hereby depose and
state:

1. That I am known by the name, FARHAN BARSI ABDULGAFFAR as

appearing in my Certificate of Live Birth;
2. That the said name has also been reflected in all of my documents and
transactions except in my Voter’s Identification Card;
3. That in the said Voter’s Identification Card, the name appearing therein is
FARHAN TOMAWIS BARSI;
4. That the name FARHAN TOMAWIS BARSI has also been used in the
filling up of the Disaster Family Access Card (DAFAC) during the Marawi
Seige.
5. That both the names “FARHAN BARSI ABDULGAFFAR” and “FARHAN
TOMAWIS BARSI” refer to one and the same person, the Affiant.
