Mr. Bern E. Montanez of legal age, single residing at ZNAC, Tampilisan,

Zamboanga del Norte. . His siblings, both of whom are employed as law enforcement officers.
Mr. Montanez completed their Bachelor of Science degree in Criminology at Jose Rizal
Memorial State University - Tampilisan Campus.

II. GENERAL DESCRIPTION OF TARGET

 The subject, a 21-year-old male with a brown complexion and a height of 5'6",
exhibits a propensity for alcohol and tobacco consumption and displays a brisk gait.

III. The team conducted Close Surveillance on Mr. Bern E. Montanez who is allegedly
selling

illegal drugs in ZNAC, Tampilisan, Zamboanga del Norte on February 28, 2024.

The agents followed the subject from the oval of the JRMSU-TC to the public market of
ZNAC. The subject first bought tobacco from Dagodog’s Sari-sari store located near the public
market. He then proceeded to give the owner a sachet of alleged drugs to the owner of the said
store. After that, the subject returned to his own residence at Subong Boarding House also
located at ZNAC, Tampilisan, Zamboanga del Norte.

IV. DESCRIPTION OF CONTACTS

Joy E. Dagodog of legal age, married and the owner of Dagodog’s Sari-sari store located

near ZNAC public market. Has medium length hair with blue eyes and ivory complexion.

V. DESCRIPTION OF BUILDING CONCERNED

The sari-sari store has open windows and is painted in bright blue and red colors.

VI. TIME-OFF AND REASON FOR TERMINATION OF SURVEILLANCE

The close surveillance ended at exactly 6:02 in the evening since the subject returned to

his own residence.
