Sir:

I am humbled and glad to notify your good office that I am tendering my retirement
from my teaching service as Master Teacher I at Lungog Integrated School, within
the jurisdiction of Narvacan North District.

I served in my teaching field for at least 37 years. And the effective date of my
retirement will be on _____________. I understand that I am required to give at
least 60 days of notice prior to my retirement date.

I am prompted to retire because of my visual disability.

I am so thankful for the great opportunity of giving service to the Department of

Education. It helped me so much in developing my professional and personal
growth, and my way of living.

Truly I enjoyed my working with my colleagues and my pupils as well. I appreciate

your sincerity to help me during my tenure of service. Surely, I will be missing
everything, but oblivion always prevail.
