SPECIFICATION FOR
1. GENERAL
 This instruction covers the general requirements for factory / shop inspection and
testing of equipment and materials (hereinafter Equipment).
 In addition to the general requirements contained in this specification, the Vendor

shall pay due attention to the specific requirements of the individual specification
concerning the inspection of each Equipment item.
 Any document which Vendor submits to Contractor for approval, clarification,

information, or any certification or approval which Contractor will issue to the
Vendor, Contractor shall report to Company for final review and approval in prior
to proceed the work.
 When there is any meeting between Contractor and Vendor, minute of meeting
shall be report to Company for information.
 Without approval from Company, Contractor/Vendor cannot proceed with the work
or inspection and testing.
 Company will need a maximum of fifteen (15) working days, excluding mailing
time, from the receipt of the Vendor document to study and accept or give
alternative decisions.
 Items of Equipment for which no individual inspection specification is issued can

be inspected and tested in accordance with this instruction and Vendor’s
standards approved by Company.
 All deviations from this instruction and related inspection specifications shall be
previously proposed to Company in writing and shall be approved by Company
before placement of the order.

2. DEFINITIONS
Words used in this instruction and related inspection specifications shall have the
following meanings, unless the context requires otherwise:
 Specification(s): often collectively means all technical specifications, drawings,

and other technical documents supplied by Company and Contractor. Also
specifications and technical documents as may be agreed and made by the
Vendor, Contractor and Company.
 Test Report: means the individual record or certificate for respective test or
inspection.
 QC Dossier: means the compiled documents of Test Report, table of contents,

other necessary documents, and the prescribed Company’s Cover Sheet.
 Contractor’s Inspector: means inspector(s) dispatched by Contractor or

inspector(s) of inspection agency (agencies) designated by Contractor.
 Company’s Inspector: means one or combination of inspector(s) dispatched by

Company and inspector(s) of the independent inspection agency (agencies)
designated by Company.
 Vendor: means a supplier or manufacturer of items of Equipment.
 Company: means LONG SON PETROCHEMICALS CO., LTD including its

personnel, representative and permitted assign.
 Contractor: means TTCL PUBLIC COMPANY LIMITED (TTCL).
 Shop Inspection and Test Plan: means the plan or program which describes the
minimum requirement of inspection and test to be carried out by vendor or
manufacturer; at shop, and/or the standard inspection activities of Contractor and
Company.

 Witness point: means a specifically identified critical step in shop manufacturing

and testing at which the work must be inspected by Contractor’s representative/
Company’s representative. The Vendor/Sub-vendor is obligated to notify
Contractor and Company a reasonable time in advance of such steps. The
Vendor/Sub-vendor may proceed with the work agrees to waiver of such witness
provided.
 Hold Point: means a point same as witness point except that the Vendor/Sub-
vendor may not proceed with the Works until Contractor’s
representative/Company’s representative has inspected it or a written notice of
waiver of Contractor or company is obtained.
 Spot witness point: means spot inspection activity during fabrication at Vendor/
Sub-vendor’s workshop. Contractor/Company reserved the right to spot
inspection. The inspection notification is not requires. (The Vendor/Sub-vendor
shall submit monthly inspection schedule activity, If Contractor/Company
required.)
 Review point: means review of inspection reports/documentation by Contractor

representative/Company’s representative.
3. CODES AND STANDARDS
 All codes and standards for the inspection of equipment shall be in accordance
with those specified in each requisition and specification.
 Any codes inspection required such as statutory inspection, government

regulation as specified in the requisitions must be arranged for any work by
contractor to obtain necessary certificates from the appropriate authority at his
responsibility and expense, unless otherwise specified.

4. CLARIFICATION OF CONFLICTS
If there is any conflict between the related specifications, codes and standards,
Contractor shall immediately consult with Company in writing for clarification. All
conflicts previously shall be approved by Company before proceeding of next step
manufacturing or inspection/test.
5. RESPONSIBILITY FOR QUALITY
 Inspection or non-inspection by Company shall not relieve Contractor/Vendor of

their obligation for quality control activities and guarantees for the workmanship or
performance of the Equipment.
 Contractor shall be responsible for inspection and testing of the Equipment,

including parts and materials supplied by Vendor/Sub-Vendors.
 Contractor shall inform and collect necessary information to Company of the

name of Sub-Vendor when the Vendor sub-contracted; Sub-Vendor’s name shall
be approved by Company in prior to be sub-contracted.
Further, Contractor shall instruct his Vendor/Sub-Vendor to comply with the

related requirements in the Purchase Order description for Equipment concerned.
Contractor shall immediately inform to Company of the following information of the

Sub-Vendor for Equipment concerned when the inspection activity is planned at
the Sub-Vendor’s factory:
 Commodity and quantity sub-order.
 Sub-Vendor name & factory location.
 Person-in-charge of the Sub-Vendor with his TEL and FAX number and email.
 Inspection activity.
 Estimated inspection date.

 Contractor shall carry out all inspections and testing specified in the purchase
documents.
 All Vendor’s assembly and fabrication procedures, including, but not limited to,
welding, nondestructive examination, heat treatment, and major repair
procedures, specified in the requisition shall be reviewed and approved by
Contractor prior to start of fabrication.
6. INSPECTION LEVEL REQUIREMENT
 In general, Contractor will carry out all inspections and tests in accordance with
the inspection level specified in Attachment 4 Inspection Level of Vendor/ Sub-
vendor.
 Contractor shall also apply the requirement of the inspection level for the
Equipment and its auxiliary in accordance with the Inspection Level List, unless
otherwise specified.
 For the Equipment and its auxiliary sub-ordered to the Sub-Vendor, Contractor
shall assign the level of the inspection for each good to comply, according to the
Inspection Level, and shall inform to Company in writing for his approval.
7. PRE-INSPECTION MEETING
 Contractor shall issue the overall inspection schedule to Company between after
engineering stage and before Pre-Inspection Meeting. This overall inspection
schedule and Inspection & Test Plan (ITP) shall be updated and informed to
Company at monthly basis.
 The overall inspection schedule shall consist of the detailed information of each
inspection activity for each Equipment individually, but not limited to the
followings:

 Date of fit-up /or joint preparation start/end
 Date of welding procedure qualification test (when applicable)

 Date of impeller over-speed test
 Date of rotor balance test
 Date of hydrostatic test
 Date of leak test
 Date of mechanical running test
 Date of performance test (when applicable)
 Date of function / sequence / simulation test
 Date of electrical test (insulation resistance, high-voltage & continuity)
 Date of factory acceptance test
 Date of final visual / dimension / quantity check
 Date of final test records review
 Date of pre-shipment inspection
 And etc.
 Contractor shall conduct Pre-Inspection Meeting(s) in advance to start the

fabrication/assembly of the Equipment to solve problems concerning the
compliance with the purchase documents, specifically inspection requirements
and related specifications. Contractor shall notice Company the Pre-Inspection
Meeting (PIM) schedule in advance at least one (1) month before meeting date
and Company reserve the right to attend that meeting. The Minute of Meeting
(MOM) shall be informed to Company for information.

 The following documents shall be issued by Contractor to Company prior to the

meeting(s);
 Vendor’s Inspection and Test Plan

 Vendor’s detail Inspection and Test Procedure
 Overall inspection schedule showing inspection items, place and time schedule
for each inspection item
 Overall production schedule
 Sub-vendor list
 Related technical documents such as calculation sheets, fabrication drawings,
WPS/PQR and etc.
 Contractor shall promptly arrange and prepare the following documents as

needed;
 Copy (Copies) of Contractor’s requisitions, drawings, specifications of the latest

revision and other documents necessary for discussion at the meeting
 List of missing documents such as specifications referenced in Company’s
requisition, but not received by Contractor
 Questionnaire about Company’s documents
 Contractor/Vendor’s drawings, diagrams, and schedules
 Fabrication/assembly procedure
 Inspection procedure including NDE procedure
 Written welding documents
 Major repair procedure
 Sample of inspection record (s) for a similar job previously done by Contractor/
Vendor
 Other documents considered necessary

8. WITNESS INSPECTION NOTICE
 Contractor shall notify Company of readiness for witness or hold point inspection
in writing at least 1 month prior to the date when the items of Equipment will be
ready for inspection.
 Contractor shall also inform Company of the definite witness or hold point
inspection date at least 2 weeks in advance of the date of the witness or hold
point inspection.
 Verbal inspection notice is not acceptable. Notice shall be sent to Company by

mail, airmail, telex, or facsimile.
 The inspection notice shall also include the map or the necessary information to
reach Vendor’s shop.
 Form of the inspection notice by mail or facsimile is shown in Attachment-1.

Contractor shall use Attachment-1 form as standard.
9. WITNESS INSPECTION
 All equipment and materials furnished and work performed shall be properly
inspected by Contractor, and shall at all times be subjected to inspection by
Company.
 Contractor’s and Company’s Inspector shall have free entry to those parts or
areas of manufacturing shops of the Vendor or its Sub-Vendors where work,
testing or inspection on the items of the Equipment is being performed.
 Contractor’s and Company’s Inspector reserve the right to monitor, audit, and visit
plants or other inspection locations with or without prior notice to the Vendor or
Sub-Vendor.

 Regardless whether the Vendor’s in-house inspection has already been

performed, the witness or hold point inspection by the Vendor for Contractor’s and
Company’s Inspector shall be performed, when the witness or hold point
inspection is mutually agreed upon.
When the hold point is specified in the requisition or agreed mutually, the Vendor
shall not proceed with the further work until the hold point is inspected by
Contractor’s and/or Company’s inspector or a written notice of waiver of
Contractor or Company are obtained.
 The Vendor, when so requested, shall provide one additional copy of the latest
approved drawings for the use of Contractor’s and Company’s Inspector prior to
the start of inspection.
 The Vendor shall furnish Contractor’s and Company’s Inspector with shop test
data, mill certificates, and other materials to verify that each item of Equipment
meets the requirements of the specified specifications.
 Copy, telex, facsimile, and telephone service by the Vendor shall be made
available to Contractor’s and Company’s Inspector.
 Vendor shall furnish Contractor’s and Company’s Inspector with inspection tools
for the inspection.
 The Vendor shall take care of Contractor’s and Company’s Inspector’s safety
during the inspection.
 All inspection record and MOM after each inspection shall be copied for
Company’s Inspector.

10. CONTRACTOR’S PRE-SHIPMENT INSPECTION
 The Vendor shall submit a complete set of manufacturer’s inspection / test reports
and quality certification of the Vendor before the Pre-Shipment Inspection
mentioned below for Contractor’s review and approval.
 When the all shop inspection and test activities have been satisfactory verified,
the Pre-Shipment Inspection shall be done with Contractor’s inspector to check
following points, but not limited to:
 Quantity, marking and tagging of the Goods including the loose supply items
 Spare parts and special tools
 Cleanliness of internal and external of the Goods
 Completion of the final QC Dossier
11. CONTRACTOR’S INSPECTION CERTIFICATE
 Contractor will issue Contractor’s Inspection Certificate to the Vendor, when

Contractor confirms the Equipment is compliance with the requirements of the
Purchase Order description, after the all quality control activities and
documentation have been completed satisfactory.
 The conditions to issue this certificate, which shall be complied by the Vendor, are
listed below:
 The final inspection completion to be verified by Contractor or his designated

representative Inspector.
 Receiving in timely the final QC Dossier prepared by the Vendor, including all
inspection and tests records and certificates, and those were verified
satisfactory by Contractor or his designated representative inspector.

 Contractor never issues the certificate until above conditions are verified
satisfaction. Contractor’s inspection certificate shall not relieve the Vendor from
his responsibility for the guarantee and the workmanship.
12. RELEASE FOR SHIPMENT
 Contractor will notify the Vendor of the inspection acceptance shortly after the
final inspection and Contractor confirms and accepts the completion of QC
Dossier prepared by the Vendor.
 Contractor shall not ship any item of Equipment without securing the specified
inspection acceptance. Contractor’s inspector shall issue the inspection release
note (form as Attachment-5) and send to Company for information; then, the
shipping preparation will be started. In case Company witnesses the inspection,
release note shall be submitted to Company for final approval before shipping
preparation.
 Any item of Equipment shipped without the specified inspection acceptance shall
be automatically considered rejected.
 Contractor will not release the Equipment if the goods:
 Do not meet Contractor’s the Purchase Order.

 Have any irremediable or injurious defects
 Poor workmanship
 Have been excessively repaired
 The Vendor have not been completed necessary documentation
 When Contractor is permitted to ship on condition to perform the following work at

the work site, Contractor shall bear all costs and expenses incurred by such work.
 Additional and/or modification work

13. PREPARATION QC DOSSIER
13.1 Unit of preparation
 The QC Dossier of individual Equipment shall be prepared for each equipment.

 The QC Dossier of unit or packaged items such as boiler units, compressor
units, etc. shall be prepared for each unit or package including subsidiary
equipments and accessories.
 The QC Dossier of materials, pumps, valves, fittings, instruments, electrical
equipment, which are not included in (1) and (2) above, may be prepared for
each order.
 In case of partial shipment, the QC Dossier may be divided for each shipment.
 The index of the QC Dossier shall be issued to Company for his approval within
8 weeks after the Purchase Order.
13.2 Forms
To prepare the QC Dossier, Contractor may use his standard forms except the
cover sheet for which the use of Company’s standard sheet is required
(Attachment-2).
13.3 Compilation
 As far as possible, A4 size (210mmX297mm) paper shall be used for the test
report sheet.
 When thickness of the QC Dossier is too thick to compile in one package, it can
be divided into two or more packages.

 In this case, Cover Sheet shall be attached to each package and serial No. shall
be given on each Cover Sheet.
13.4 Construction of QC Dossier

Note: Table of content may be omitted in case that the content (Test Reports) is
less than 10 sheets.
As far as possible, the construction of QC Dossier shall be in the following order:
 Material certificates
 Nondestructive examination records
 Visual inspection certificates
 Dimensional inspection records
 Hydrostatic test records
 Leak test records
 Other test records such as heat treatment records
13.5 Company’s cover sheet
 Contractor shall fill in the cover sheet as illustrated in Attachment-2.

 The Cover sheet shall bear the signature of Contractor’s representative before
submittal of copies of QC Dossier to Company.
 Contractor shall insert Contractor’s inspection certificate next to the cover sheet
13.6 Test report
 Each test report sheet shall have the verification signature by Contractor.
 Each test report sheet shall have the following data or information in addition to
signatures:
 Test data and/or results with acceptance criteria
 Date of test

 Identification of components tested
14. DOCUMENTATION
 Incomplete, inaccurate or illegible documentation shall be the cause for rejection

of items of Equipment until such time as satisfactory rectification has been
effected.
 Contractor shall submit to Company Three (3) copies by air courier and electronic
file (in MS-word, excel, Acrobat format) for QC documents such as inspection
procedure and welding procedure specification as required in the respective
inspection specification.
 Unless otherwise specified, all these documents shall be prepared in English.
 Contractor shall be requested to submit these documents prior to fabrication.
14.1 Submittal of QC Dossier
 The QC Dossier, which consists of the complete set of the inspection, test
records, reports and certificates shall be submitted by electronic file for
Company’s final confirmation within one (1) week after all quality control
activities are completed satisfactory or in advance to the Pre-shipment
inspection prior to the shipment whichever comes earlier, in principle.
 In addition to above, when the partial shipment is applied, each electronic file
shall be sent to Company for his final confirmation before the delivery of the
partial shipment goods from the factory in accordance with the same manner
mentioned above. However the final QC Dossier shall be submitted to
Company Three (3) copies by air courier and electronic file (in MS-word, excel
or Acrobat format)
14.2 Inspection and test procedure and ITP
 Contractor shall issue the inspection & test procedure for approval of Company.
This procedure shall consist the explanation of the each quality control activity

with its acceptance criteria for the Equipment concerned. This procedure shall
be submitted for Company’s approval within 8 weeks after the date of the
Purchase Order or 6 weeks in advance to the first witness inspection date
whichever comes earlier.
 Contractor shall develop Inspection and Test Plan (ITP) as format in
Attachment-3 for approval of Company based on the job specific requirements,
which are specified in the relevant specifications, and Contractor’s ITP, when it
is attached in the requisition, related to the Equipment concerned.
 Contractor’s ITP shall summarize all quality control activities of the Equipment
concerned (Including the activity of the Sub-Vendor) but not limited to the
followings:
 Hold or Witness point/Record review point of Contractor/Company related to
each activity.
 Applicable specification, Code and Standard related to each activity
 Type of each inspection & tests records or certificates related to each
activity.
 These ITP shall be submitted for approval of Company within 4 weeks after the
Purchase Order.
 Without approval of Company for this ITP, Contractor cannot proceed with the
inspection and tests and arrange the witness of hold point inspection for
Company.
 Any changes or deviations from original requirement of Contractor are subject
to a written consent by Company.
 Contractor/Vendor may, however, propose alternatives equivalent to or more
stringent than the original requirements, if appropriate under Company
approval.
14.3 Material testing record & certificate
 Contractor shall propose his plan of the Material Testing Record & Certificate
for all commodities included in Contractor’s scope of supply in his quotation.

 Company will check and comment it when it is not appropriate.

 The requirement of the document formality of the material testing certification
shall be referred to the requirement of EN10204 (DIN 50049 ‘Inspection
documents for delivery of metallic products”) even though the Vendor is not
European party.
 Unless otherwise specified, the following type of the material testing record &
certificate shall be provided, as minimum Company’s requirement:
 Type 3.1B certificate: for all pressure retaining parts and all metallic parts
contacts with process fluid
 Type 2.2 certificate : for others
14.4 Non-conformance control
 When the non-conformance is found during inspection and tests or fabrication,

Contractor shall issue the non-conformance report (NCR) for Company’s
approval immediately within one or two calendar days after the date of the non-
conformity is found out. (Contractor is requested to use the designated form
which developed by Contractor under Company’s approval as a standard.)
 In this event, the NCR shall include a detail description of the condition of the
non-conformance and recommended & appropriate corrective action.
 Without written approval of Contractor concerning the NCR, the Vendor cannot
proceed with the corrective action.
14.5 Major weld repair
 When the defect found is the major defect, the Vendor shall issue the detailed
defect map and the weld repair procedure (which describes the work step of the
repair, applicable NDE and its extent before and after weld repair, and
WPS/PQR) for Contractor’s approval.
 Without the approval of Contractor for these documents, the Vendor shall not
proceed with the repair work.

 The entire major weld repair shall have appropriate records which shall consist
of the defect map, the material certificates of the weld consumable, the NDE
records, the heat treatment records, and so on with traceability.
Note: The major defect is defined as follows, unless otherwise specified.
 The depth of any cavity exceeds 20% of the wall thickness, or 1 inch
(25mm), whichever smaller, or
 The area of any cavity greater than 10 inch² (65 cm²)
15. SPECIFIC FACTORY / SHOP INSPECTION REQUIREMENT
In additional, the specific inspection requirement below shall be mentioned in

procurement procedure and carried out by inspection work.
15.1 Mechanical
 NPSH Test for pumps, Positive Material Identification for all alloy and
stainless steel procurements and dismantling inspection after shop
mechanical running test shall be clearly specified in the inquiry and
inspection shall be carried out.
 Bulk materials shall have passed the quality assurance system of the
manufacture and provided with material certificate
 After completion of all inspections and tests and before shipping,
austenitic steel vessel shall be chemically cleaned at welds and Heat
Affect Zone (HAZ)
15.2 Instrument
 Shop inspection requirement of instrument shall be in accordance with

Annex C, section 6 INS-023: Instrument Inspection and Test Plan and
other relevant specification in Annex C, section 6.

Attachment-4: Inspection Level of Vendor / Sub-vendor
1. GENERAL
Contractor shall use the system of coding given here below in proposing his level of planned inspection coverage at home
office.
Contractor shall consider key factors such as complexity, special material, unusual processes, safety, risk, operational
reliability, quality history of Vendor or Subcontractors, ease of replacement, site location and delivery schedule when
proposing the inspection level. Contractor shall substantiate its recommended level of inspection coverage which shall be
incorporated into the Vendors or Contractor quality plans and shall be reviewed by Company.
Contractor shall take note and take into account any arrangements that may be necessary for Company or their appointed
Third Party Inspectors to review and be in attendance on all major equipment items and packages.
The following lists the four (4) levels of inspection coverage:
Level 0 - Quality assurance inspection and testing by Manufacturer / Vendor;
Level 1 - Perform the visual inspection prior to packing and also final inspection of Equipment and packing prior to shipment;

Level 2 - Includes as a minimum, inspection meetings, one or more unspecified, “in- progress”, surveillance type visits or
observation, all witness and hold points, final inspection and release for shipment or final acceptance;
Level 3 - Same as level 2 above except that ”in-progress” surveillance inspections shall be on a regular basis (weekly,
monthly, etc.); and resident inspectors to continuously monitor the Works, when required, shall be identified on ITP.
