We, the Grade 10 students of Holy Infant Academy Anda Inc. We venture into a study
entitled "The Effects of Self-esteem on the Academic Performance”. We humbly asked
you to spare a little of your very precious time to answer this set of questions. Rest
assured that our response will be dealt with utmost confidentiality.

“Strongly Agree” with a score of five (5) means that the respondent has the most
positive idea that there is available support when needed.
“Agree” with a score of four (4) means that respondent has a positive idea that there is
available support when needed.
“Neutral” with a score of three (3) means that the respondent has unsure/equal idea that
there is an available support when needed.
“Disagree” with a score of two (2) means that respondent has a negative idea that there
is available support when needed.
