Department of Education extends its resources and looks for varied ways to cater
your needs and to adapt to the new system of Education as a fortress of Learning
Continuity Plan. One of the probable solutions is the use of Teacher-made

You are reading the English 5 Quarter 1 Alternative Delivery Mode (ADM)
Module 1 on Filling-out Forms Accurately.

The creation of this module is a combined effort of competent educators from


different levels and various schools of the Department of Education-Cebu Province.
In addition, this module is meticulously planned, organized, checked and verified
by knowledgeable educators to assist you in imparting the lessons to the learners
while considering the physical, social, and economical restraints in the teaching
process.

The use of Teacher-made Educational Module aims to surpass the


challenges of teaching in a new normal education set-up. Through this module, the

Essential Learning Competencies based on the K-12 curriculum competencies, to

WHAT I NEED TO KNOW

Have you tried to enroll in school? Have you used the library or have
received money transfer? Did the school, librarian or teller ask you to fill out
some forms? If you did, then you have the idea of what this module is about.

A form is a printed or typed document with blank spaces where


important information about you like name, birthdate and address are
written.

Forms are documents that you have to fill out as a requirement for
you to be accepted to a school, allowed to use the library or open a bank
account. These are important because the information you write in the
forms tell about you. There are many types of forms that you may fill out.
There are deposit and withdrawal slip from the banks. There is a bio-data
that is required when you apply for a job. In school there are enrolment
forms, membership forms if you want to join in an organization and voting
forms when you choose your leaders.

This module shall teach you how to fill out important forms
accurately.

At the end of module, you should be able to fill out the following forms
completely and accurately: school forms; deposit and withdrawal slips; bio-
data; election ballots.

After studying this module, you should be able to:

FILLING OUT FORMS

Do you know what forms are? Have you seen or filled out a form
before? Was it easy or difficult to answer?

Filling out forms is an important skill that needs to be learned.


Forms need a lot of information. This information is to be written correctly
and accurately in spaces provided.

A form often requires you to write name, address, date and place of
birth, names of your parents, education background, work and so on.
Writing the information in an accurate way is a skill that has to be learned
or else you may provide wrong or incomplete information.

Forms have many uses. They are used for gathering information, for
registration, identification, enrolment and certification.

Surname – is your family name

Gender – tells if you are a boy/male or a girl/female

Here are some helpful tips in filling out forms:

1. Read the form carefully before you write anything.


2. Make sure that you write complete and necessary information or
details.
3. Find out if you are asked to write block letters or block capitals in
boxes.
4. If an item does not apply to you, leave it blank.
5. If there is an item that you do not understand, ask for help.
6. Review the entries before submitting the form.

Let’s Practice:
When you grow older and you want to apply for a job, you need to
prepare and answer some forms. One of these is a biodata. A Biodata is a
common form that asks you to write personal details in addition to
educational and professional experience. It includes detailed personal
information such as date of birth, gender, religion, race, nationality, marital
status, residence, parents' names, names of your children and siblings,
languages spoken, etc.

Have you seen a biodata answered by your parents, brothers or sisters


when they applied for a job? If not, you may request your mother to buy a
biodata sheet so she can help you understand the parts and words that
require answers. Read the activity below and find out if the form is
