DEED OF UNDERTAKING

I, KRISHIA MAE G. BULACAN, fourth (4th) year student, taking up Bachelor of Science
in Public Administration at Dalubhasaan ng Lungsod ng Lucena, and I, DR. RUTH MARIE
A. TUBOG, faculty of the same school, herein referred to as TRAINEE and PRACTICUM
ADVISER, respectively, do hereby declare, manifest, and undertake the following actions
to wit:

THAT the trainee shall undergo On-The-Job Training (OJT) in OIC, Community
Affairs for a minimum of two hundred (200) hours taking into consideration the trainee’s
allowed schedule starting .

THAT, the trainee is not entitled to any remuneration allowance or reimbursement

of any kind from OIC, Community Affairs, since the OJT is solely for the purpose of
completing a requirement of the Bachelor of Science in Public Administration curriculum
prior to a student’s graduation.

THAT the trainee shall abide the policies, rules and regulations set by OIC,
Community Affairs, its employees and the trainee’s immediate supervisor deemed
necessary for the OJT;

THAT the trainee shall not disclose any confidential information obtained during
the training;

THAT the trainee shall be subjected to a periodical performance evaluation based

on the guidelines set by the school; and

THAT we hold OIC, Community Affairs free from any liability for the duration of
the On-The-Job Training.

In connection with the ongoing deployment of our fourth-year students for their On-the –
Job Training, we would like to seek assistance from your office that they be allowed to
work in your office as an OJT for two hundred (200) hours as required in our program for
Bachelor of Science in Public Administration.

The purpose of this apprenticeship is to expose and immerse our students to actual work
or office environment in a government or private office or agency to observe its best
practices as well as familiarize themselves with the necessary jobs and skills required
in an actual work place.

Given the opportunity to work in your office, we shall appreciate the guidance and
assistance you will extend to our On -the -Job Trainees.

We hope that this request will be favorably considered in the interest of preparing our pool
of competent professionals.

TRAINING AGREEMENT

Our agency/institution/establishment thru the undersigned Department Head/Trainer

agrees on the following training provisions with the Dalubhasaan ng Lungsod ng Lucena.

1. The BSPA student trainee, Krishia Mae G. Bulacan shall render office
apprentice/practice in the following schedules:

DAYS TIME
M-F 8: 00 AM – 5:00 PM
2. That the above mentioned BSPA student trainee will perform activities/work or
during his/her specialization in our agency/institutions/establishment.

3. That the above mentioned BSPA student trainee is not to work overtime or during
holidays, Saturday and Sunday unless officially requested and arranged.

4. That the above mentioned BSPA student trainee shall be required to present end
paper evaluation of this practicum program.

5. That our agency/institutions/establishment thru its department head/cooperating

office supervisor shall comply with school evaluation requirements for the student
trainee.

6. That our agency/institution/establishment shall exercise all necessary diligence

keeping the trainee safe and sound while under our supervision and control.
