1. Purpose : To establish a safe work procedure for working in a confined space as to protect
the workers and others against risks to their health while working in a confined
space.
4. Definition : “confined space” means any chamber, tank, manhole, vat, silo, pit, pipe, flue or
other enclosed space, in which —
(a) dangerous gases, vapours or fumes are liable to be present to such
an extent as to involve a risk of fire or explosion, or persons being
overcome thereby;
(b) the supply of air is inadequate, or is likely to be reduced to be
inadequate, for sustaining life;
“entry” means ingress by a person into a confined space which occurs when
the person’s head passes through an opening into the confined space;
“hazardous atmosphere” means an atmosphere where —
(a) the level of oxygen in the atmosphere is not within the range of
19.5% to 23.5% by volume;
(b) the level of flammable gas or vapour in the atmosphere is 10% or
more of its lower explosive limit; or
(c) the levels of toxic substances in the atmosphere exceed the
permissible exposure levels as specified in the First Schedule to the
WSH (General Provisions) (Reg 1);

5. Responsibility :
5.1 Project Manager (PM)/ Person In-Charge (PIC) and Authorised Manager shall ensure that the
confined space is safe for work before any work to commence in the confined space.
5.2 Confined Space Safety Assessor (CSA) shall ensure that no work is carried out or is in progress
without the prior permission of the PM/ PIC/ Authorised Manager.
a. ensuring that entrants and confined space attendants are briefed on the safety and health
precautions to take prior to confined space entry and work;
b. authorising entry into confined spaces;
c. ensuring that entrants and confined space attendants adhere to entry procedures;
d. rescue equipment and appointed rescue personnel are available when confined space
work is to be carried out;
e. implementing control and preventive measures to manage all identified hazards are
implemented;
f. applying the entry permit and have it endorsed by the Authorised Manager before
commencement of confined space work;
g. terminating the entry permit after completion of work.
5.4 The main role of the entrants is to do work in confined spaces. Entrants are responsible for:
a. following entry and work procedures when carrying out work in confined spaces;
b. carrying a portable gas/vapour measuring instrument for continuous monitoring of the
atmosphere in the confined space for the full duration of their work;
c. inform the confined space attendant of any unsafe atmospheric conditions or when any
emergency situation arise; and
d. inform co-workers to evacuate from the confined space should the portable gas/vapour
instruments’ alarm be activated due to unsafe atmospheric conditions.
6. Procedure :
6.1 Planning
6.1.1 Before allowing the workers into a confined space, the Supervisor / CSA shall check the
following by completing the Confined Space Checklist:
Workers are aware of the hazards and are trained on wearing the PPE (i.e. air
breathing apparatus, safety harness) and emergency proceedings.
Oxygen level within 19.5 % to 23.5 % by volume.
The level of flammable gases, vapours or dusts are present at a concentration less
than 10% of their lower explosive limit (LEL).
The toxic airborne level does not exceed the permissible exposure level.
Testing to be conducted throughout the entire area with a properly calibrated
instrument.
The test interval should not exceed by 4 hours
Where applicable, at least one worker in the confined space be equipped with a
suitable gas monitoring device
Materials (which include tools) that will give rise to toxic / hazardous/ flammable gas
or fire are removed or rendered safe to use in confined space;
Adequate ventilation and lighting (at least 50 lux) will be provided during the course of
work;
Adequate means of ingress/egress are provided.
Availability of suitable belts, ropes and reviving apparatus and oxygen (these should
have been thoroughly examined once a month)
To review and assess the need to continue with the entry into a confined space on a
daily basis
6.1.2 If the above conditions are satisfied, the Supervisor / CSA shall report to the PM/ PIC who
shall then issue the permit to allow the workmen to start work in the confined space.
6.2.3 Display “DO NOT ENTER” Tag and “BEFORE YOU ENTER TAG”
6.3.1 The Supervisor shall ensure proper local exhaust ventilation or general ventilation is
provided at all times to remove excess CO2, oxides of nitrogen etc, which may arise during
operation in confined space like welding, cutting, burning, solvent, paint coating etc.
6.3.2 The Supervisor shall assign an attendant who shall be stationed outside the confined
space. The attendant shall keep track of identity / number of entrants. Attendant shall
monitor the activities in and around the space and shall make effective / continuous
communication with the entrants.
6.3.3 The PM / PIC shall formulate an emergency response team, first aid team and shall keep
them alert during the work in progress.
6.3.4 The workers shall observe the following while carrying out work in the confined space:
Never smoke in confined space
Always maintain communication with attendant while working in a confined space
Never use defective electrical connection / tools which may cause fire

6.3.5 The Supervisor / CSA shall ensure that no work is in process or started without valid
permit to work approved by the PM/ PIC.
6.3.6 In case of any incident, attendant outside the confined space shall immediately inform the
emergency response team/ PM/ PIC. PM/ PIC shall take the appropriate measure as per
emergency preparedness and response procedure.
6.3.7 After the job had been completed, the workers shall report to the SS/ CSA.
6.3.8 The Supervisor / CSA shall keep the test records i.e. O 2 content, presence of toxic or
hazardous gas etc (if any).
6.4 Training
All supervisors, entrants and confined space attendants must be trained prior to being appointed to
work in confined spaces.
6.4.3 The Supervisor / CSA shall arrange training and regular site safety promotional activities
for the workers and continuously monitor the situation:
6.5.1 Ensure there is always sufficient supply of suitable breathing apparatus, safety harness
and ropes, suitable rescue equipment and reviving apparatus kept readily available.
6.5.2 Ensure all these supplies of rescue equipments are properly maintained and examined by
a competent person on a monthly basis .
6.5.3 Keep records of inspection results for rescue apparatus and equipments for at least 2yrs
6.5.4 Participate in drill for rescue in confined space once a year and kept drill record including
the time, date of drill, personnel involved, a short description and evaluation of the drill.
6.6.1 During the period of confined space entry and work, the following rescue equipment shall
be made available;
a. A tripod with a retrieval system capable of hoisting casualty out of the confined space;
b. Rescue stretcher or harness, capable of strapping the casualty to allow him to be
hoisted safely out of the confined space;
c. Breathing apparatus for rescue personnel.
