AFFIDAVIT OF THE EXPLANATION

,MARIO V.RAFAEL of legal age, Fillpino, with accordance with the law, do hereby dispose and
say, to wit:

1. That I am the Board of Director Chairman and an authorized representative of BACTAS FARMERS
AGRICULTURE COOPERATIVE (BFAC)

2. That the said cooperative is duly registered with the Cooperative Development Authority-
Naga Extension office with a Registration No. 9520-1050000000047193.

3. That based on records, the said cooperative found to be non-compliant due to non-submission
of following mandatory reports ANNUAL REPORT for the year 2022.

4. That we were not able to submit the said mandatory reports due to still on hold so that we can't
access on the CAIS.

5. That the said failure to submit is unintended the only for the reason adverted to above;

6. That the herein promise to not to commit the similar violation for its future compliance with the
mandatory reports of the authority.

7. That I am executing this affidavit in order to attest to the truthfulness submission of the
required reports with the Cooperative Development Authority-Naga Extension Office and for
whatever legal purposes this may serve.
