INITIAL REQUIREMENTS

1. Submit an introduction video in a well-lighted room, free from clutter and wearing a
professional attire. (2-3 minutes length)
Your video will include:
 Full name
 Where you are from
 What subjects do you teach
 Your teaching experience
 Why do you love teaching?

2. 5 minute Lesson Demonstration

___ 1. Official Transcripts from ALL colleges and universities attended (one original set).
Official Transcripts should be ordered and mailed to address above.
___ 2. Course by course evaluation of foreign credentials (Josef Silny) (two originals, one for
employer and 1 for employee). Required only if your transcripts are from an
international university
___ 3. DS-160 Template Form (This will be emailed to you to fill and return to me)
___ 4. Resume (1 copy)
___ 5. Diplomas (1 copy notarized)
___ 6. Two (2) passport size photos 2x2
___ 7. Bio page of a valid passport; must show the validity dates of the passport and all passports
must have at least six months beyond the intended 3 year stay in the US (2 photos)
___ 8. Birth Certificate (1 copy) Candidate must also have an original to travel with.
__ 9. Marriage Certificate (1 copy)
___ 10. Praxis Tests (1 copy) Visit www.praxis.com (Most teachers will take this in their first
year at work)
______11. SEVIS 1-901 fee receipt (http://www.ice.gov/sevis/i901 ) (1 copy). This you will go
online and pay ($220) after you are hired and I have sent you the DS2019.
___ 12. Verification of all teaching experience after you graduated with you Bachelors. Please
give start and end date for each teaching job you held. (1 original from each
experience) Please use the Employer’s designated form.
__ 13. Physician’s statement of good health (1 original). There are no special forms
___ 14. Three professional recommendation letters on official stationary, “ To Whom It May
Concern” in separate sealed envelopes, with candidate’s name on the envelope. (Not from
friends & family)
___ 15. Employer job application (1 original)
______16. Your Local National Teacher Certificate or License (1 copy, please have it
notarized)
___ 17. Criminal background check, including finger-print card from your country’s police
Headquarters (1 original and 1 copy). /NBI
___ 17 A. Background check form filled in by you and your signature witnessed.
___ 18. Signed WDTT Contract (original and 1 copy).
__ 19. Signed Deed of Undertaking re funds for first 60 days (original and 1 copy).
___ 20. Signed Disclosure Statement. (original and 1 copy).
