AFFIDAVIT

I, Sangars Madhukar Rao, son of Venkat Rao, aged 63 years and residing at Sahid
Lal Singh Street, Dashmesh Nagar, Kachha Thandewala Road, Sri Muktsar Sahib,
solemnly affirm and sincerely state as follows:
1. I declare that I hold Passport No. M544173, issued from Nagpur, which
expired on 14th December 2002. In 2022, I applied for the reissue of a new
passport under File No. AS1066099723822. However, due to the omission of
my old passport details in the application, the passport was not issued, and I
received a closure letter Ref. No. FCL/324411332/23 dated 13th September
2023, advising me to reapply, mentioning the old file number from the
Regional Passport Office (RPO) in Amritsar.

2. Subsequently, on 8th January 2024, I submitted a new application for the

reissue of my passport under File No. AS2079055600224. Unfortunately, I
inadvertently failed to include my old File Number AS1066099723822, as
instructed in the closure letter issued by the RPO office.

3. I acknowledge my oversight and sincerely apologize for the mistake made in

my new application. I request your understanding and kindly ask for your
assistance in reissuing my passport as soon as possible.
I apologize once again for any inconvenience caused and thank you for your
attention to this matter.
