REPORT on Floor Crane Accident Investigation
With reference to the telephonic call from ……………….. & ………………..–Both From M/s …………, , we ( Mr. Albal B.
S. as a Consultant Safety & Quality Systems, “Chartered Engineer, Valuer Plant & Machinery ) reached to M/s ……………...), at
…………….. ( Research Centre ), Building within …………….., Near …………, Whitefield, Bangalore. We met Mr.
……………………... Discussed the subject in detail and we all went to the Site where the FLOOR CRANE is
installed. Our observations are noted Below,
1. In certificate motor Capacity is given as 0.75KW, Pl check, This Low capacity M/c will be heated
2. Rope dia is 12mm, As per IS 2266:2002, min breaking force 6500Kg considering any type of Rope.
Considering Factor of Safety=4, Load that can withstand by 12mm Rope becomes 1625Kg & Safe.
3. Coming to the Design part : Spool Side Plates are around 4mm thick Plate. The Place where Rope
is coming out of Spool for Locking is having Sharp Corners & above the Base Drum of the Spool.
While Winding & Releasing the “Rope Wires” in the “Rope Strands” are rubbed against edge of
the Spool Side Plate. Since the side Plate Opening is Sharp, not rounded off, the “Rope Wires” will
cut one after the Other. So Rope will fail within very short period. May be within 5 to 10days.

4. Immediate Solution : a) Every 2 Hours Check the Temperature of the motor, b) Provide the
shed for Motor, so that sun heat will not add to machine heat.
5. Immediately change the Position of Rope Entry Position. As given sketch.
6. Every 2 days, unwind the Rope fully and check for damage, Record to be maintained for the Same.
After the Project is completed the before installing in other site take it for Future Action,
