I. BRIEF DESCRIPTION OF THE INCIDENT AND RESPONSE:

At around 2236H of December 30, 2023, the MDRRM Office received a report from a
concerned citizen through hotline call regarding a motorcycle accident at along National Road
Brgy. Calaocan, Rizal, Kalinga. In immediate action, the MDRRMO rescuer conducted an
urgent response on the said incident. The team dispatched at 2240H and arrived at incident
around 2244H.

At the scene, the victim was spotted lying in front of domeng’s panciteria. The victim was
responsive but he had wounds on his right ankle, facial abrasion and lacerated wound at right
hand. The victim smells alcoholic. The rescuers quickly applied first aid to stop the bleeding and
clean the victim's wounds before transporting him to Juan M. Duyan Memorial District Hospital
for further assessment, care, and treatment. The victim was endorsed at the hospital around
2258H.

The victim was identified as Patrick Gazzingan, a 18-year-old male residing in Brgy. Santor.
He was allegedly under the influence of alcohol while operating his motorcycle, leading to a loss
of control and a subsequent fall on the side of the road.
