Summary of Observations: - Smith Emery Laboratories (SEL) Quality Assurance Inspector Shernell Concessio (QAI)
arrived at Livio Building Systems Pvt Ltd, at the job site met with Sundar Pattnaik (HRS Manager) &
Mr. Kiran (QC Lead).

Upon arrival at the facility, QAI inspected the following documents:

Welding and Fit-up inspection:
QAI observed that fit-up of the following components is completed and ready for inspection.

VISIT SUMMARIZATION

QAI performed a random fit-up and dimensional verification of the above listed components with reference to the
the provided drawings and the tolerances specified in the Livio Quality Manual. Welder was identified as Mukesh
Kumar (welder ID: W-MS) to perform the welding.
Welding was performed utilizing the Flux Cored Arc Welding (FCAW) process. QAI verified the consumables, filler
wire was observed as AWS A5.20/ASME SFA-5.20 E71T-1C, ESAB India Limited, Batch no.: BGX1732399. No rust,
moisture or any contamination on the filler wire was observed. Shielding gas was identified as 99.9% Carbon
Dioxide (C02), calibration of the welding machine was verified and was observed to be current.

HIGHLIGHT

While performing random fit-up check and visual inspection,


QAI observed the following.
 Plate Fitup measurements met requirements.
 Bottom end of the Pillars was welded as per code for the suggested length and width & throat (7 mm)
 QAI noticed Start stop had sharp edges and did not meet code, corrections were made after being
highighted.No slag or porosity detected.
 Pillar (C2-02-01, C2-02-02, B2-01-01) did not meet fillet size requirements for leg size, these were highlighted
to Sundar, Kiran & Dinesh and asked for corrective action before welding was complete.

The necessary corrective measure was taken by Livio QC. Unacceptable welds were corrected either by welding or
grinding. QAI performed a random visual inspection of the identified locations and were observed to be satisfactory
for welding.

Welding was observed to be performed utilizing FCAW process. QAI randomly verified the welding parameters and
were observed in compliance with the WPS. Meanwhile, it was noticed that the flow meter appeared not
functioning correctly and possibly hindering the flow of gas during welding. This was relayed to Livio QC. Corrective
measures were observed to be taken and the welding machine was replaced with another machine (# WM-01) to
continue the production welds. Livio HSR Manager will notify Smith Emery for inspection after completion of
welding of all the parts.
