CONDUCT COMPETENCY ASSESSMENT

I. BEFORE THE ARRIVAL OF THE TRAINEES
I already checked the assessment area, availability of supplies and materials,
the condition of tools and equipment and ensure that the environment is
suitable for the assessment and the candidates are relaxed and not
apprehensive.
Make introduction to the panel.
II. ARRIVAL OF THE TRAINEES
Good day everybody!
May I request you all to stand up for a prayer? (Amen)
My name is Lara Jane T. Lobo, I am an accredited competency assessor for
Animal Production Poultry-Chicken NC II. (Wear an Assessor I.D)
 I would like to acknowledge the presence of Miss Jesila A. Ocariza, the
Assessment Center Manager of Provincial Training Center and
, our representative from TESDA Provincial Office. You do not have to
worry about their presence. They are just here to oversee the conduct of
competency assessment and to check if the conduct is in accordance
with the prescribed competency assessment methodology and
procedures. They will not assess you.
III. ATTENDANCE
Before I start, let me first check the attendance. Please present to me your
admission slips and bring a valid ID once your name is called.
(compare signature of candidates on admission slip against the signature
on the attendance sheet)
I have also with me the Data Privacy consent form. As you can see in the
assessment center, we are covered with CCTV. This is to record the proceeding
of the assessment to ensure quality outcome. Please write your name and
signature for us to proceed with the assessment.

IV. SELF-ASSESSMENT GUIDE

I have here with me your accomplished application form and self-assessment
guide. This is an indication that you know and can perform all the units of
competencies enumerated for Animal Production Poultry-Chicken NC II.

1. Today, I am going to administer a competency assessment Animal

Production Poultry-Chicken NC II. The purpose of our assessment is to
determine the level of your competencies and to measure your
underpinning knowledge and skills.
2. These qualification’s core competencies are Maintain poultry environment,
Brood and grow chicks, Perform pre-lay and lay activities and Trim beak.
3. For your personal necessities, lavatories are located at the left side of this
building, and you can also take your meals at our own canteen located at
 the lobby. However, during the assessment, you are not allowed to leave the
premises of the assessment center.
4. If you have questions don’t hesitate to ask me because I am more willing to
assist you Like if there’s a need for you to clarify matters concerning
assessment, you may raise your hand and I will assist you now. However,
once the assessment has started, please refrain from asking questions
because it is no longer allowed, and cellphones are also strictly prohibited.
5. In case, you have questions regarding the assessment results, you have the
right to appeal. You can submit your appeal to the TESDA Provincial Office
for the assessment center and assessor accredited.
6. During the actual assessment, please observe safety as you perform the
different tasks.
7. In case of accidents, please call my attention immediately so that we can
provide first aid, our first aid kit is located on the right side of this building.
8. Is there anybody here who has any severe health conditions? None, all very
good.
9. With regards to the result of your assessment, we observe strict
confidentiality. The result will be made available to the authorized personnel
and to the institution where you had an actual training.
10. After the assessment, if you are not found competent, don’t worry about
it because you can be reassessed after a month or when you feel confident
enough that you have acquired the competence.
11. But if you are found not yet competent with two consecutive
assessments, you will be advised to enter the training again.

13. I will now distribute this Specific Instruction, please read carefully and if
you have any clarification, please raise it now so that we can address it
immediately before we start.
14. Since there are no more questions, I will now assign your corresponding

  You are following the standards in performing the skills needed in Animal
Production Poultry-Chicken NC II. You were also very clear your answer in
oral questioning.
 Congratulations for a job well done! You are already competent.
 Please sign the competency assessment result summary (CARS).
 Your National Certificate/Certificate of Competency in Animal
Production Poultry-Chicken NC II will be sent to the email address you
provided in few days. Check your Spam/Junk folders if it doesn’t appear in
your inbox. Use A4 size paper to print it and have it dry sealed at the
provincial Office from Monday to Friday, 8-5 pm
 Claim your CARS at the AC manager’s office later. Ok!

22. (For not yet competent, Candidate # 2)

perform again?

 Based on the standards of the qualification and the evidence that I have
gathered during your performance, I found out that you are not yet
competent, you have to review/ practice the activities related to the
competency requirements of the Qualification further.
 You can come back after a month for reassessment or when you feel
confident enough that you have acquired the competence. Don’t worry you
will only be reassessed on the part of the activity that you were found to be
not yet competent.
 Ok, so please affix your signature here, and I will submit this first to the
Assessment Center Managers office and you may claim it there once there is
an advisory from this office, then bring this competency assessment result
summary (CARS) once you want to apply for reassessment after a month or
when you feel confident enough to perform the part that you fall off.
