OBJECTIVES ACTIVITIES

A. PRE-EMPLEMENTATION PHASE
1.Re-echo about the result of KADA network training to the
MHO and Pop.Com Officer.
2. Coordinate to and collect baseline data such as capacity
on: Physical Structure/Room/building, Human Resource:
KADA Network service providers and capacitation, data on
adolescent’s program form the MHO, DSWDO, PNP, Schools
and other GOs/NGOs.
3. Prepare initial stakeholder’s analysis
4. Prepare drafted/sample of KADA Network Ordinance
5. Prepare communication letter to members of Local
Health Board (LHB) and stakeholders identified in the initial
analysis
6. Conduct LHB meeting with the following
agenda/activities:
a. Finalize the Stakeholder’s Analysis and identify
KADA Network members
b. Revisit PPAs involving Adolescent’s Health in
Agencies/Offices/organization/committees and for
possible source of fund in implementing the said
PPAs
c. Present the baseline data/information gathered
d. Finalize the drafting of KADA Network ordinance
e. Finalize list of Adolescent’s PPAs
f. Identify KADA Center/Facility
g. Identify/create referral system/flow of activities
h. Recommend for the inclusion of the identified PPAs
in the municipal/barangay level AIP
i. Revisit the level of capacitation of Human Resource
 involve in the implementation of Adolescent’s
Program
B. IMPLEMENTATION PHASE
7. Include identified PPAs with corresponding fund
allotment in the municipal/barangay AIP/Budget.
8. Do out sourcing of fund from NGOs, Private Businesses
and individuals
9. Adopt the Municipal KADA Network and KADA Ordinance
in the Barangay Level
10. Provide KADA Center/Facility with complete supplies
and equipment/with good amenities
11. Establish KADA Network in the municipal and barangay
level
12. Conduct survey on the level of Adolescent’s knowledge
on Adolescent’s Program and how and where to access
them.
13. Implement Adolescent’s PPAs
C. POST IMPLEMENTATION PHASE
14. Collect/Consolidate/Submit Reports:
a. Reports from Barangay KADA Network Center
b. Municipal Level: Reports from
offices/agencies/NGOs involved in the
implementation of Adolescent’s Program
c. Consolidated and submit to Provincial KADA
Network Center
15. Conduct monitoring and evaluation:
a. Program Review-semi-annual/annual
b. Conduct/Prepare Situational Analysis with
corresponding recommendations to the identified
gaps/issues and concern
c. Conduct visitation to all KADA Centers/Facility
d. Conduct regular inventory of supplies/equipment
