I. DEFINITION:

A. Successfully assumes responsibility and demonstrates constructive initiative in school and/or

community activities.

B. Illustrates complete involvement and dedication of time and effort by showing a willingness to
spend time in school and/or after school to reach/set goals.

NOTE: Leadership qualification is not restricted solely to those holding office.

II. PROCEDURE

A. Candidate will submit Leadership form to one faculty member of her/his choice who has not yet signed a
survey to verify candidate’s qualifications in areas of character and service.

B. Faculty members should return Leadership forms to the NHS advisor.

III. TEACHER RECOMMENDATION

I have carefully read the traits listed below and have checked them in effort to evaluate and substantiate that
the student named above be considered for induction into the National Honor Society.
