SUPPLEMENTAL PETITION

PETITIONER, Renato C. Villanueva, through counsel, to this Honorable Court respectfully sets
forth that:

1. The above-entitled case is a petition for issuance of Second Owner’s Duplicate Copy of
TCT -49610 registered in the name of Federico R. Aguilar Sr. married to Anicia Yu Aguilar;

2. On 05 October 2011, Angelina Revoltar , the person who was in possession and custody
of the aforesaid TCT-49610, executed an Affidavit of Loss which clearly stated the circumstances under
which the Owners’s Duplicate Copy was lost;

3. On 06 October 2011, petitioner Renato C. Villanueva submitted a copy of said Affidavit

of Loss to the Registry of Deeds of Legazpi City and had the instrument duly registered and had a
memorandum thereof made on the Original Transfer Certificate of Title. A certified machine copy of the
Affidavit of Loss submitted to the Registry of Deeds of Legazpi City is attached and marked as Annex
“A”;

4. Moreover, to fully comply with the requirements of the law, petitioner Renato C.
Villanueva secured a certification that the real property taxes for the lot covered by TCT-49610 have been
paid. A certified machine copy of the certification from the Office of the City Treasurer of Legazoi City is
attached and marked as Annex “B”;

Wherefore, premises considered it is respectfully prayed of this Honorable Court that this
supplemental petition be admitted.
