Please find attached Construction Self-Assessment Checklist.

This checklist should be completed once a month by the project management of each contractor.
Compliance to this process is a critical safety indicator for Unilever.
Please submit your completed form to RHDHV by the 25th of each month.

Please go over each statement in the form and if there is a deviation against the standard or
room for improvement indicate this in the form by marking the °YES” box in the “do you propose
action” section.
Also include a short description of what action you will take to ensure compliance.
This can be done in the “remark” section.

Submission of this form can be done via the regular submittal procedures of RHDHV.
If you have any questions, do not hesitate to contact Odri Amir, RHDHV SHE manager for further
