5. fill the TEMPLATE:

1. well owner's information, check in project folder then open permit folder
2. well location, no need fill township.

10. water found at encountered depth, if not found then untested

12. just check disinfected no
13. save image of google earth with the pin then attach
14. just fill date work completed

17. validate, enter well number. press signature box, sign as tony haddad,
password is 123456
18. make validated pdf, and also print normal pdf as _OC (original/owner's
copy idk)
