A NARRATIVE REPORT ON THE SCHOOL LAC ON ORIENTATION ON THE CHECKING OF

SCHOOL FORMS AND CONDUCT OF YEAR – END RITES 2023

The Department of Education (DepEd) hereby prescribes the DepEd order No.11, s. 2018 the standard

process and protocols in the preparation, evaluation, and updating of school forms and conducted at the end of

every school year. This policy aims to provide a reliable assurance mechanism of learner information, ensure the

quality and timeliness of school reports, and reduce the resources spent for clerical and records management and

likewise the issuance of an order updating the Multi-Year Implementing Guidelines on the Conduct of the K to

12 Basic Education Program End-of-School –Year Rites (DepEd Order no. 009, s. 2023) as an amendment to

and reissuance of DepEd Order 2, s. 2020 which provides guidance to both public and private elementary and

secondary schools nationwide in the annual conduct of EOSY rites.

Thus, the School Guidance Designate and Grade 6 Adviser of Labit Elementary School, Mrs. Marissa

M. Servito organized and conducted a school LAC session on Orientation on the Checking of School Forms

and Conduct of Year-End Rites on June 14, 2023, to update/orient teachers on the implementing guidelines and

policies in the preparation, checking and evaluation of school forms. And also to guide and update teachers on

the Multi-Year Implementing Guidelines on the Conduct of the K to 12 Basic Education Program End-of-School

–Year Rites.

The LAC session started with the registration and opening program, followed by the LAC proper.

Competent resource speakers discussed their assigned topics and teacher-participants actively participated in the

discussion.

Indeed, the LAC session was a success due to the concerted effort of School Head, the LAC organizer

and Teachers.
