Community Quarantine.

Ms. Gayle Baligod has been stranded in this Barangay since the imposition of the Enhanced Community
Quarantine. Ms. Baligod has undergone 14 day home quarantine. Ms. Baligod is not in the Barangay’s list of
probable, suspect, contact, confirmed cases.

This Certification is issued for the issuance of the Medical Clearance Certificate of Quezon City.

Quezon City strictly enforces the guidelines and protocols of the Enhanced Community Quarantine.

Will it be okay kung Brgy Health Centers na Based on the NTF Reso, the obligation to issue the Medical
ang mag issue since under daw ng CHO and Certification for the Travel Authority of LSI is under the City/Mun
operations nila? Health Office.

pwede din ba na barangay certification na lang Based on the NTF Reso, the obligation to issue the Medical
din for those intending to leave para hindi na Certification for the Travel Authority of LSI is under the City/Mun
magulo sa city level Health Office.
