GENERAL CONTROL MEASURES

Personnel must be trained before WAH


 Select a suitable method when WAH
 Properly use prevention of falling when WAH from 2 m or more

DEFINITION
Scaffolds: Be a temporary structure to perform work at
height such as: maintenance jobs, construction..., and can also
be used to make walkways and lifting/lower equipment from
height.
Purpose of this procedure is to ensure safety during
erection/dismantling and use of scaffolding in BSR

GENERAL SAFETY REQUIREMENT
Note: All certificates are issued to participants and to follow scaffolding standards such
as BS 1139-5973-5974-5975, EN 12811, US OSHA: 3150, AS/NZS 4576 or equivalent.

Certificate of Primary scaffolding course: is issued to participants of short-term


scaffolding training course according to Vocational Education Law.

Certificate of Level 1: is issued to participants who have completed a training course


on the erection & dismantling of scaffolding

Certificate of Level 2: is issued to participants who have completed a training course


on calculation, design and supervision of scaffolding works

GENERAL SAFETY REQUIREMENT
SCAFFOLDING MATERIAL:
 Damaged materials which do not meet the scaffolding
standards, must be marked up and stored separately and
promptly moved out of storage and / or working area

 Scaffolding materials must be placed and arranged neatly on


the firm foundations, convenient for scaffolding works and not
obstructing walkways, exits or other emergency rescue
equipment

SCAFFOLDING MATERIAL:
 Scaffolding materials must have a certificate of origin
by the manufacture, in good condition.

 Steel tube scaffolds must be covered at both ends


with plugs. Plugs are made of plastic or rubber
materials

SCAFFOLDING MATERIAL:
 Base plates are manufactured and used according to the
manufacturer's instructions

 The sole plates must be made of a firm material and ensure to


withstand the calculated loads

+ The minimum area of sole plates (for heavy duty/medium


duty/light duty scaffolds) is not less than 1.000 cm2 (No any dimension
is less than 22 cm)

+ For special duty scaffolds: The size of the sole plates must be
determined by the designer.

Planks
 Strong and in good condition to withstand the
calculated load

 Not heavy rust or sticking lubricants such as oil,


grease or other corrosive chemicals

 The gap between planks must not exceed 2.54cm

GENERAL SAFETY REQUIREMENT
Scaffolding design:
 All scaffolds in BSR must meet the requirements of design,
erection, use, dismantlement as stated in the technical
documents and/or certificate of origin by the manufacturer.
Do not erect, use or dismantle scaffolding without the above
documents.

 Before erection of scaffolding, all scaffolds must be designed


and meet the requirements of the dept./section that requires
the use of scaffolding.

 BSR allows contractors to apply other international scaffolding


standards for scaffolding works in BSR. it ensured that the
standards meets the requirements of the scaffolding laws and
regulations in this procedure

GENERAL SAFETY REQUIREMENT
Scaffolding design:
 Scaffolding design drawings must contain the following contents (but not limited to):

GENERAL SAFETY REQUIREMENT
Scaffolding design:
 Scaffolding design drawings must contain the following contents (but not limited to):

- Height, width, braces, working platforms and access of scaffolding. Location, size of
bay expansion, location of horizontal bracing points for scaffold which have a height
exceed three (03) times  the minimum base width.

- Material, size of base plate/sole plate and wheels (if any)

- Main anchor locations (for hanging scaffold)

- Name of designer, reviewer and approver of design dept./contractor

- Name of reviewer and approvers of Division/dept. of BSR

GENERAL SAFETY REQUIREMENT
Erection/dismantling of caffolding :
 Do not carry out scaffolding works in bad weather
such as thunderstorms, darkness, rain or strong
winds of level 5 or higher (equivalent to 8 m / s)

 The foundation of scaffold must be firm and


capable of withstanding calculated load. In
particular, pay attention to the locations with open
ditchs/sewers, excavation, erosion and on the
fixed platform.
 Put warning sign and tape when working
scaffolding works

GENERAL SAFETY REQUIREMENT
Erection/dismantling of caffolding :
 Install horizontal bracing or base extension for scaffold
which have a height exceed three (03) times
 the minimum base width

 Along with to the vertical direction, the height of 4 m or


every two working platform must have at least one
horizontal bracing point and/or base extension ≥3h

 The pairs of cross braces must be erected at the lowest


base of the working platform to prevent deformation.
Scaffolders should NOT stand on clamps or braces to h
working

GENERAL SAFETY REQUIREMENT
Erection/dismantling of caffolding :
 Before erection of scaffolding, it is required to
install temporary planks (when there is no
working platform yet) at suitable locations of
each floor for scaffolder standing to work to next
floors

 Temporary planks are not working platforms and


are only allowed to be used during the erection
and dismantling of scaffolding

GENERAL SAFETY REQUIREMENT
Base plate and sole plate:
 The base plate/sole plate is used to distribute load on
foundation. It must be install the base plates for
scaffolds placed on the fixed platform (Grating) and
the base plates should be placed immediately on the
support/structures of the floor.

 Scaffolds placed on the ground or pavement, both the


base plate and the sole plate must be used for all
standards of scaffolds

GENERAL SAFETY REQUIREMENT
Base plate and sole plate:

 The base plates must be placed at the center of the sole


plates, not allowing the use of planks as a the sole
plates.

 Do not placed the base plates on the unstable sole


plates such as bricks, broken/cracked wood, etc.

GENERAL SAFETY REQUIREMENT
Working platform:
 The width of the working platform is at least 0.5 m. The
load placed on the working platform should be evenly
distributed and planks must be clamped tightly to the
ledgers by specialized clamps (do not use zinc wire) ≥ 0,5 m

 The planks must be tightly placed together and the maximum


allowable gap between planks is 2.54 cm and the slope of
working platform is not more than 7 °

 The plank must overlap on ledgers and the overhang length


from ledgers between 0.15 m and 0.25 m. The overhang sides
shall be separated from the working platform by handrails

GENERAL SAFETY REQUIREMENT
Handrail and toe board:
 Hnadrail must be installed on all open sides from 2 m above
the ground or work surfaces below

 The height of top Handrail is from 0.9 m to 1.15 m above


the working platform. Intermediate Handrail should be
placed at a distance of 0.45 m to 0.6 m from Top Handrail.

 For fabricated frame scaffolds, cross braces may be used


instead of intermediate handrail when the two-point
intersection is at least 0.5 m and not more than 0.75 m from
the working platform.

GENERAL SAFETY REQUIREMENT
Handrail and toe board:

 All scaffolds must be installed toe board for


preventing falling objects, the minimum height of toe
board is equivalent to the width of a plank.

GENERAL SAFETY REQUIREMENT
Handrail and toe board:
 The two-way access from the ladder to the working platform must always be closed after every
access to ensure the prevention of falls, similar to the contiguous space between the working
platform and the internal ladder , it must also covered to protect against falls

GENERAL SAFETY REQUIREMENT
Ladder and access:
 The maximum height of a ladder is 6 m. The distance between
the rungs is from 0.23 m to 0.36 m, the width of steps is not be
shorter than 0.3 m

platform is 0.9 m

 It is not allowed to stand on the top three (03) steps of the ladder

GENERAL SAFETY REQUIREMENT
Ladder and access:
 It is allowed to use scaffolding tubes connected to the structure of
scaffolding to set up the safe access between the floors of scaffold
but must comply with the distances of rung of ladder

 At a height of 5 m or more from the base plate of scaffolding or scaffold


is placed at height of 5 m or more above the ground, if installing an
outer ladder, there is a risk of falling to the ground, it is mandatory to
install ladders inside of base of scaffold and ladders must be installed at
different intervals between working platforms

GENERAL SAFETY REQUIREMENT
Ladder and access:
 The slope of the ladder is recommended not to exceed
75 ° with the horizontal. For mobile steel tube
scaffolds, ladders must be installed inside of base of
scaffolding

 Ladders must be firmly fixed and always


maintain three points of contact when using the
ladder. Do not use cross braces as a means of access
on the scaffolding

GENERAL SAFETY REQUIREMENT
Safety net:
 Prevent people/materials/tools from falling into the working areas below, normal
size of net is about 4 m x 3 m. The width of safety net is more than 2.5 m on all
sides from the edge of the working platform

 The load of safety net ensures that it can withstand the maximum load as
required. The distance from the nearest working platform to the safety net must
not exceed 4.5 m

 The installation of safety net will be considered, decided in JSA and ensure the
following requirements.

+ Do not contact with hot surface;

+ Do not shade the light and ventilation for the surrounding area.

GENERAL SAFETY REQUIREMENT
Lifting/Lowering by manual fixed pulley :
To ensure safety during lifting/lowering work, the following safety
requirements must be complied with:

Fixed Pulleys and related equipment must be inspected and sticked sticker

 The pulley must be firmly anchored to the fixed structure and in a suitable

location for the lifting man who must wear a safety harness when working.
The weight of load must not exceed 30kg/1time/person

 Material/equipment must be arranged neatly and secured tightly before

GENERAL SAFETY REQUIREMENT
Lifting/Lowering by manual fixed pulley :
For ensuring in safety during lifting/lowering works, the following safety
requirements must be complied with:

 Installation of pulley includes the main components such as: Main supporting
bar, reinforcing bar and slip-resistant device.

 For fixed pulleys connect to the structures of scaffolding: Scaffold must be


securely braced and/or put a suitable counterweight to ensure absolute safety
when lifting/lowering.

 Put warning sign/tape at the lifting/lowering area and only carry out when this
area is safe.

GENERAL SAFETY REQUIREMENT
Lifting bag:
 Lifting bags are made of durable, strong material. On the body
of the bag, the safe working load (SWL-kg) must be specified.

 Lifting bag must be checked to ensure safety before use and


only allow to lift/lower up to 30kg/1 time.

 Do not store bulky materials and exceed the safe working load
of the bag. Before lifting/lowering, the bag must be tightened
to prevent the falling of material during transportation

GENERAL SAFETY REQUIREMENT
Material basket:
 The specification of manufacturer on lifting bag must
clear and placed at easily identifiable location with
the following content: the name of manufacturer,
manufacturing year, serial number, lifting load in
kilograms (kg) and/or tons ( t)

 The lifting basket is made of metal material, the


main supporting structures are welded together, the
bottom is flat and there is no gap.

 The lifting basket must have enough 4 lifting lugs

GENERAL SAFETY REQUIREMENT
Material basket:
 The lifting basket must be tested and certified by a
third party

 To ensure safety when transporting or Design


lifting/lowering, the equipment and materials inside sample
the basket must be neatly arranged and lower than
the height of surrounding sides before
lifting/lowering. The guide rope must be tied to
control the moving direction during lifting/lowering.

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Steel tube scaffold:
 Be Scaffolding consists of steel pipes and connectors such
as standards, ledgers and braces.

 The pair of horizontal braces at the bottom floor should be


placed close to the base plate of the scaffold and installed
at a distance not exceeding 20 cm from foundation/fixed
platform. The overhang length of horizontal braces from
standards is between 0.15 m and 0.25 m.

 Cross bars should be fixed to the point of intersection as


close as possible to prevent deformation, a maximum
distance of 20 cm is recommended. 20 cm

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Steel tube scaffold:
 The standards are allowed to use connectors and to ensure the locations
of the connecting points must be placed askew on the same floor. Do not
use connectors on the ledgers
 Scaffolding components on a scaffold must have the same specifications

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Mobile scaffold:
 The height of mobile scaffold not exceed three
(03) times of the minimum base width.

 For the mobile aluminum alloy scaffolding: if


the minimum width of the base is less than 1.2
m, the height of scaffold must not exceed two
(02) times this width

 The ladder of mobile scaffold must be installed


inside of scaffolding base

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Mobile scaffold:
Wheel:
 All wheels of the mobile scaffolding must ensure to
withstand four (04) the calculated loads. It is strictly
forbidden to use the wheels of unknown origin.

 The wheels shall be caster wheels (not the air tire) with a
minimum diameter of 0,1 m, unless special design is
required by other material.

 The caster wheels must be firmly fixed to the legs of


scaffold and have brakes or locks to prevent the movement
of scaffold.

 Gather scaffold to a safe place and lock the wheels when


not in use

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Mobile scaffold:
- Only use mobile scaffolds on flat surfaces and enough
capable of withstanding the calculated loads;

- Ensure the stability of scaffold during moving. There


must be no obstructions on the road;

- Do not allow people, materials, hand tools or other


equipment on the working platform during moving
scaffold

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Hanging scaffold:
The hanging scaffolding is usually erected in places where it cannot be erected
or if erecting scaffold from the ground will be uneconomical.
 The main supporting structure of hanging scaffolding systems must be
capable of withstanding at least 1.5 times the calculated load of the scaffold

 Before erection, it must carefully survey the scene to select the appropriate
location (space, main anchor points, ...). After that, the designer will peform
detailed designs to ensure safety for later scaffolding works

 For this type of scaffold, it is required to install auxiliary tubes (T-Step) to


support scaffolders during the erection and dismantling of scaffold

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Fabricated frame scaffold:
The fabricated frame scaffolds are quickly assembled together by frame and cross
braces. It often used in construction such as construction scaffold, modular scaffold,
etc. or the type of scaffold is made of aluminum alloy which BSR equip to use

 The installation of this scaffold must comply with the manufacturer's instructions
and meet the requirements of the dept./section of the use of scaffold. The working
platforms must be fully covered before working. It must be specified a safe working
load on the body of scaffold

 The Frame and components must have equivalent specifications. To ensure safety,
it is allowed to reinforce this type of scaffolding by using the materials of steel tube
scaffolding (tube, toe board) to enhance the rigidity of the scaffold and preventing
falling objects. For this type of scaffolding, it is allowed to replace the toe boards by
safety net to prevent the falling object.

SAFETY REQUIREMENTS FOR OTHER SCAFFOLDS
Fabricated frame scaffold:

 From a height of 5 m or more, ladders and/or stairs must be installed inside of


the scaffolding base. The slope of the stairs does not exceed 60 ° from the
horizontal surface;

 When placing a scaffolding frame on another scaffolding frame, it is necessary


to brace and locked together prevent the collapse of scaffold

phải được tiến hành trước khi sử dụng.

 Inspect the frame and components periodically and before use to replace
damaged materials .

INSPECTION AND SCAFFOLD’S TAG
Inspect before use:
 After completing the erection of scaffolding, the relevant parties must inspect the whole scaffold
and hang tag before use (According to the form BSR-HSE-PRO-029 / F-002)

 After insecting, The form (including periodic or unscheduled inspections) must be signed by
relevant parties and attached to PTW (allow to use a copy) before approval.

 Do not modify scaffold without approval.

 Weekly (no more than 7 days), scaffold must be inspected and signed on the
form BSR-HSE-PRO-029 / F-002 before attaching it to PTW to continue
scaffolding works at site

 If it have any changes to the scaffolding structure, the scaffold must also be
inspected and re-tagged before use.

 After inspecting, The green tag must be attached to scaffold with full
information such as load, inspection date, name and signature of inspectors.
